The level of dependence of indices of insulin resistance for BMI (waist circumference) and their values for metabolically healthy individuals

Abstract:

Úvod: Inzulínová rezistence je stále aktuálnějším tématem. Je definována jako stav, kdy normální hladina inzulínu v plasmě vyvolá nižší biologickou odpověď v organismu. Cílem této diplomové práce bylo zjistit závislost indexů inzulínové rezistence (QUICKI, HOMA IR) na BMI (obvodu pasu) u vybrané skupiny respondentů a zároveň zjistit hodnoty těchto indexů u metabolicky zdravých jedinců. Metodika: Teoretická část práce je věnována obecně tématu inzulínové rezistence a metabolickému syndromu. V praktické části byly zpracovány údaje 213 respondentů z ordinace v Hradci Králové. Z hodnot glykémie a inzulinémie nalačno byly vypočteny indexy inzulínové rezistence a zjištění jejich závislosti na hodnotách BMI a obvodu pasu bylo provedeno pomocí korelační a regresní analýzy. Podle definice metabolického syndromu byli respondenti rozděleni na dvě skupiny a pomocí popisné statistiky a statistických testů u nich byly zhodnoceny hodnoty indexů inzulínové rezistence. Výsledky: Z výsledků šetření vyplývá zjištění statisticky významných lineárních závislostí indexu inzulínové rezistence QUICKI na hodnotách BMI (p<0,0001), indexu HOMA IR na hodnotách BMI (p=0,0038) a indexu QUICKI na hodnotách obvodu pasu (p=0,0007). Závislost indexu HOMA IR na obvodu pasu nebyla v tomto šetření prokázaná jako statisticky významná (p=0,0820). Hodnoty indexů inzulínové rezistence byly u osob bez přítomnosti metabolického syndromu vyšší (index QUICKI), nižší (index HOMA IR) než u osob s přítomností metabolického syndromu, tento rozdíl byl statisticky významný (p=0,0095). Výskyt inzulínové rezistentence byl u jedinců bez přítomnosti metabolického syndromu nižší než u jedinců s přítomností metabolického syndromu a tento vztah byl také statisticky významný (QUICKI p=0,0011, HOMA IR p=0,0001). Závěr: Tímto šetřením byly prokázány a popsány vztahy mezi indexy inzulínové rezistence, základními antropometrickými veličinami a složkami metabolického syndromu.

Abstract:

Introduction: Insulin resistance is an increasingly topical theme. It is defined as a condition when a normal insulin levels in the plasma induces a lower biological response in the organism. The aim of this thesis was to determine the dependence of the indices of insulin resistance (QUICKI, HOMA IR) on BMI (waist circumference) in a selected group of respondents and also to determine the values of these indices in metabolically healthy individuals. Methods: The theoretical part is devoted to the general topic of insulin resistance and metabolic syndrome. In the practical part were processed data about 213 respondents from the clinic in Hradec Kralove. From the fasting plasma glucose and fasting insulin were calculated indices of insulin resistence and the detection of their dependence on the values of BMI and waist circumference was carried out using correlation and regression analysis. According to the definition of the metabolic syndrome, respondents were divided into two groups and using descriptive statistics and statistical tests were evaluated the values of the indices of insulin resistance. Results: The survey results to determine statistically significant linear dependence of insulin resistance index QUICKI on the values of BMI (p<0,0001), index HOMA IR on the values of BMI (p=0,0038) and index QUICKI on the values of waist circumference (p=0,0007). Dependence of the index HOMA IR on the waist circumference was not in this investigation demonstrated a statistically significant (p=0,0820). The values of indices of insulin resistance were in persons without the metabolic syndrome higher (index QUICKI), lower (index HOMA IR) than in individuals with the metabolic syndrome, this difference was statistically significant (p=0,0095). The incidence of insulin resistantance was in individuals without the metabolic syndrome lower than in individuals with the metabolic syndrome, and this relationship was also statistically significant (QUICKI p=0,0011, HOMA IR p=0,0001). Conclusion: In this investigation were demonstrated and described the relationship between indices of insulin resistance, basic anthropometric variables and components of the metabolic syndrome.
