The Delivery After Previous Cesarean Section

Abstract:

Bakalářské práce se zabývá postoji a přání žen k vedení druhého porodu, který následuje po císařském řezu. Práce se skládá z části teoretické a praktické. Teoretická část informuje o základních faktech císařského řezu, jeho historii, podmínkách provedení, indikacích a možných komplikacích. Vysvětlena je zde analgezie a anestezie používaná u císařského řezu. Popsán je také fyziologický porod s jeho mechanizmy. Dále jsou zde uvedeny dvě možnosti vedení porodu po předešlém císařském řezu se všemi jejich výhodami, nevýhodami a riziky. Praktická část je zpracována kvalitativní metodou výzkumu. Pomocí rozhovorů s deseti ženami, které mají za sebou porod císařským řezem a jsou znovu těhotné, je zjišťován hlavní cíl této práce, tj. zda mají tyto ženy přání rodit vaginálně či by raději rodily opět císařským řezem. Z rozhovorů jsou vytvořeny případové studie, ze kterých byla provedena souhrnná analýza. Následně rozhovor s ženami proběhl i po porodu, kdy ženy zhodnotily, zda jim byla umožněna jejich volba porodu a zda se naplnila jejich očekávání. Z výzkumu vyplývá, že většina žen preferovala ukončení těhotenství pokusem o vaginální porod, ale jen polovině žen byl tento pokus umožněn. Bohužel jen jedné z nich se podařilo úspěšně vaginální porod dokončit. I když se drtivé většině žen porod nepovedl dle jejich přání, všechny ženy ale hodnotí svůj porod kladně a nemají na něj negativní vzpomínky. Výsledky výzkumu přesvědčují, že porodní asistentka může prospět v těhotenské poradně s podáváním informací těhotným ženám po císařském řezu. Ty, které nemají kontraindikace vaginálního porodu, více motivovat k pokusu o přirozený porod a těm, které je mají, být oporou a dobře je připravit na opakovaný císařský řez. Tento výzkum by měl také více motivovat porodníky, aby císařské řezy indikovali pouze při opodstatněných důvodech.

Abstract:

This Bachelor thesis is about opinions of women, which already gave a birth, towards giving a second birth via caesarean section. Bachelor thesis is composed of theoretical and practical part. The theoretical part informs about elementary facts of caesarean section, its history, conditions of successful execution, indications and possible complications. The analgesia and anesthesia used at caesarean section are explained too. Physiological childbirth is described with all its mechanisms. Furthermore, both options of leading a childbirth after previous caesarean section with all advantages, disadvantages and risks. The practical part is described with qualitative research method. Main task of this thesis is researched by interviews with ten women which all of them gave a birth with caesarean section in past and they are pregnant again. Furthermore, if they are willing to give a birth vaginally or again via caesarean section. Case studies were created from interviews and used for pooled analysis. Interviews with experimental group were done again after childbirth, where women were able to rate if they were allowed to choose way of giving a birth and if their expectations were fulfilled. The research shows that most of women preferred end of pregnancy by vaginal childbirth but only half of them were allowed to. Unfortunately only one woman was able to successfully finish vaginal childbirth. Even if most of women weren’t able to give a birth as they wished, they don’t have a negative memories regarding giving a birth. Research also shows that midwife can help in pregnancy counseling by providing information to pregnant women after caesarean section. Help motivate to give a birth naturally those women who don’t have a contraindications of vaginal childbirth, be a support to women who have them and help them to be ready for repeated caesarean section. Results of this research should also motivate obstetrician to perform caesarean section only when well founded
