The assessment of sucking patterns of full term infants and preterm infants

Abstract:

Cíl: Cílem práce bylo posoudit vzorce sání donošených a nedonošených novorozenců pomocí škály The Neonatal Oral-motor Assessment Scale (NOMAS) v podmínkách praxe ČR. Tato škála rozlišuje 3 vzorce sání podle orálně motorických funkcí novorozence. Vyvinula ji Nizozemka Marjorie Meyer Palmerová v roce 1993. Metodika: Výzkumný soubor tvořilo 63 novorozenců, z toho bylo 32 donošených a 31 nedonošených novorozenců. Výzkumné šetření probíhalo od 1. 7. do 31. 12. 2013 ve Fakultní nemocnici v Ostravě a ve Slezské nemocnici v Opavě formou dotazníku, který zahrnoval vybrané demografické údaje a škálu NOMAS. Výsledky: Na základě šetření jsme identifikovali vzorce sání u donošených a nedonošených novorozenců. Zjistili jsme, že donošení novorozenci mají funkční vzorec sání dle NOMAS, a proto je možné je krmit ústy hned po narození. Nedonošení novorozenci mají neorganizovaný vzorec sání, a proto se musí krmit s přihlédnutím na jejich zdravotní stav a stabilitu vitálních funkcí. Dále jsme prokázali, že děti s funkčním vzorcem sání vypijí více mléka než děti s abnormálním vzorcem sání a vzorec sání závisí také na hmotnosti dítěte. Závěr: Škála NOMAS je využitelná v podmínkách praxe České republiky k hodnocení vzorců sání novorozenců a tím jejich připravenosti na orální krmení.

Abstract:

Aim: The aim of the thesis was to assess the sucking patterns of full-term newborns and preterm newborns using The Neonatal Oral-motor Assessment Scale (NOMAS) in the conditions of practice in the Czech republic. This scale distinguishes 3 sucking patterns according to the oral-motor functions of newborn. The scale was developed by the Dutchwoman Marjorie Meyer Palmer in 1993. Method: The research group consisted of 63 newborns, 32 of whom were full-term infants and 31 were preterm infants. The research was carried out from 1. 7. to 31. 12. 2013 at the University Hospital in Ostrava and at the Silesian Hospital in Opava in the form of a questionnaire, that included selected demographic data and the NOMAS scale. Results: Based on the research, we have identified the sucking patterns of full-term and preterm newborns. We found out, that the full-term newborns have a functional sucking pattern according to the NOMAS and therefore it is possible to feed them by mouth immediately after birth. The preterm newborns have an unorganized sucking pattern and therefore they have to be fed with regard to their health status and stability of vital functions. Furthermore, we have demonstrated that children with functional sucking pattern drink more milk than children with abnormal sucking pattern and that the sucking pattern depends also on the weight of the child. Conclusion: The NOMAS scale is applicable in the conditions of the practice in the Czech republic for the evaluation of newborn sucking patterns and thus their preparedness for oral feeding.
