Nutritional status and self-sufficiency of elderly people planned surgical on the digestive tract

Abstract:

Cíl: Cílem výzkumu bylo zjistit a zdokumentovat nutriční stav a soběstačnost seniorů před plánovanou operací na zažívacím traktu a posoudit závislost mezi vznikem pooperačních komplikací a předoperačním stavem výživy. Dále pak porovnat změny stavu nutrice a soběstačnosti v momentu propuštění se stavem před operací u stejné skupiny pacientů a posoudit závislost mezi stavem nutrice a úrovní soběstačnosti. Metodika: Prospektivně formou dotazníku byla získávána data seniorů, kteří podstoupili plánovanou operaci na zažívacím traktu. Stav výživy byl hodnocen standardizovaným dotazníkem Mini Nutritional Assessment (MNA) a dále byly opakovaně odebírány biochemické parametry výživy (celková bílkovina, albumin, prealbumin). Úroveň kognitivních funkcí byla hodnocena Mini-Mental State Examination dotazníkem (MMSE) a soběstačnost byla hodnocena standardizovaným dotazníkem Activity Daily Living (ADL). Dotazník byl doplněn o základní charakteristiky pacientů (pohlaví, věk), typ onemocnění (zhoubné, nezhoubné), délku hospitalizace, datum operace, datum propuštění a výskyt pooperačních komplikací. Získané výsledky byly analyzovány a statisticky testovány. Výzkumný soubor tvořil vzorek 42 respondentů. Výsledky: Ze 42 seniorů zařazených do studie bylo 18 mužů (43 %) a 24 žen (57 %). Průměrný věk v soboru byl 78 let. 15 seniorů (36 %) bylo předoperačně v pásmu normální nutrice, 21 (50 %) bylo v riziku malnutrice a 6 (14 %) bylo předoperačně malnutričních dle standardizovaného dotazníku MNA. U poloviny pacientů byl pooperační průběh spojen s výskytem komplikací. V šesti případech (29 %) se jednalo a jedinou pooperační komplikaci, v 15 případech (71 %) byly u jednoho seniora popsány dvě a více pooperačních komplikací. Mortalita byla 9,5 %. Pouze u 10 % seniorů byla předoperačně použita nutriční intervence. Výskyt pooperačních komplikací v našem souboru statisticky významně souvisel se zhoršeným stavem výživy prokázaným jak laboratorními parametry (celková bílkovina, albumin a transferin) tak zjištěným pomocí standardizovaného dotazníku MNA. Při propuštění byly výživové parametry seniorů horší, ale s výjimkou hladiny albuminu rozdíly nedosáhly statistické významnosti. Zhoršený stav výživy předoperačně statisticky významně souvisel s nižší soběstačností seniora a soběstačnost seniorů měřena ADL testem při propuštění byla statisticky významně horší než před operačním výkonem. Výskyt pooperačních komplikací vedl ke statisticky významnému zhoršení soběstačnosti seniorů při propuštění. Závěr: Předoperační vyhodnocení stavu nutrice a soběstačnosti u seniorů je nástrojem, který umožňuje jejich cílenou nutriční přípravu. Toto opatření pak může vést ke snížení výskytu pooperačních komplikací operovaných seniorů, zlepšit pooperační průběh a minimalizovat negativními důsledky operace včetně zhoršení jejich soběstačnosti.

Abstract:

Aim: The aim of the study was to analyze and writedown the nutritional status and self-sufficiency in geriatric patiens scheduled for elective gastrointestinal surgery. The relations between preoperative nutritional status and postoperative complications were evaluated, the nutritional status and self-sufficiancy before surgery and before patient´s discharge was compared and relation between nutritional status and self-sufficiency in our group of geriatrics patients was assessed. Methods: The data of geriatrics patiens scheduled for elective gastrointestinal surgery were collected prospectively using a special questionnaire. Basic parameters of patiens (gender, age, BMI), diagnosis (malignant, benign), selected laboratory parameters of nutritional status (total protein, albumin, prealbumin, transferin), length of hospital stay and postoperative complications were recorded. Mini Nutritional Assessment (MNA) form was used for the identification of nutritional status, Mini-Mental State Examination (MMSE) form was used to screen cognitive impairment and Activity Daily Living (ADL) form was chosen to assess the geriatrics self-sufficiency. The obtained data were analyzed a statistically tested. Finally 42 geriatric patiens were included in the study. Results: There were 18 men (43 %) and 24 women (57 %) included in the study. The average age was 78 years. Preoperative sum of the Mini Nutritional Assessment score distinguishes between adequate nutritional status (15 seniors ? 36%), patiens at risk of malnutrition (21 seniors ? 50%) and geriatrics in malnutrition (6 seniors ? 14%). The postoperative course was complicated in 21 cases (50%). One complication was recorded in six cases (29%), multiple complications were observed in 15 cases (71%) respectively. The mortality rate was 9.5%. Only 10% geriatrics recieved preoperative nutritional support. There were statistically significant differences in postoperative complications in group of geriatrics with poorer nutrition status measured by laboratory tests (total protein, albumin, transferin) or evaluated by Mini Nutritional Assessment form. The selected laboratory parameters of nutritional status were worse before patient´s discharge but except albumin they did not reach statistical significance. Poor nutritional status before surgery was significantly related to worse self-sufficiency in the geriatrics and self-sufficiency before discharge assessed by Activity Daily Living (ADL) form was significantly worse than self-sufficiency before surgery. Postoperative complications statistically significantly worsened the self-sufficiency of elderly patiens. Conclusions: Preoperative nutrition status assessment and self-sufficiency evaluation provided a valuable tool for targeted preoperative nutritive support in elderly patiens scheduled for elective gastrointestinal surgery. Decreased incidence of postoperative complications, faster recovery and suppression of negative consequences of surgery including impaired self-sufficiency are the awaited results.
