Nursing Diagnosis in Older Adults

Abstract:

Úvod: Sestra v klinické praxi zajišťuje ošetřovatelskou péči prostřednictvím ošetřovatelského procesu, jehož součástí je ošetřovatelská diagnostika. Cílem ošetřovatelské diagnostiky je vyvodit závěry týkající se specifických potřeb jedince a naplánovat efektivní péči. Z důvodů demografických změn je aktuální se zabývat standardizací diagnostického jazyka v ošetřovatelské péči u seniorů. Cíl: Cílem této práce bylo zjistit frekvenci výskytu vybraných mezinárodních ošetřovatelských diagnóz dle NANDA International u seniorů v institucionální péči. Dalším cílem bylo doplnění diagnostického procesu použitím standardizovaných škál, které vedly ke zjištění stavu výživy a soběstačnosti seniorů v institucionální péči. A současně také zjistit specificitu a senzitivitu použitých standardizovaných škál pro stanovené ošetřovatelské diagnózy. Metodika: Jednalo se o kvantitativní výzkum. Byl vytvořen speciální záznamový arch, který obsahoval výběr ošetřovatelských diagnóz dle NANDA International (třída Příjem potravy, Aktivita/cvičení a Sebepéče) a měřící škály pro doplnění diagnostického algoritmu (Škála pro hodnocení stavu výživy - MNA a Test základních všedních činností podle Barthelové). V rámci stanovení ošetřovatelských diagnóz byl využit diagnostický algoritmus dle NANDA International. Výzkumný soubor byl sestaven z 200 osob v institucionální péči (zdravotnická a sociální zřízení). Průměrný věk činil 76,7 let (směrodatná odchylka byla 8,7). Minimální věk byl 60 let a nejvyšší dosažený věk činil 94 let. Mužů bylo 75 (37,5 %) a žen 125 (62,5 %). Výzkumu předcházelo získání souhlasu etické komise, managementu vybraných institucí a daného respondenta. Data byla zpracována statisticky. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky a diskuze: Všechny stanovené výzkumné cíle práce byly splněny. Ošetřovatelské diagnózy dle NANDA International ze tříd Příjem potravy, Aktivita/cvičení a Deficit sebepéče je vhodné využít v ošetřovatelské péči u seniorů, a to jak v oblasti zdravotnické, tak sociální. Na základě diagnostického algoritmu byly nejčastěji nalezeny ošetřovatelské diagnózy, které souvisely s nedostatečnou výživou a deficitem sebepéče. Diagnostický algoritmus dle NANDA International je vhodné doplnit standardizovanými měřícími nástroji. Škála pro hodnocení stavu výživy může být použita pro objektivizaci ošetřovatelské diagnózy Nevyvážená výživa: méně než je potřeba organismu, jelikož u této diagnózy bylo dosaženo stoprocentní senzitivity. Test základních všedních činností podle Barthelové je vhodný pro objektivizaci ošetřovatelských diagnóz ze třídy Deficit sebepéče, u kterých byla dosažena výborná míra senzitivity i specificity. Domníváme se, že Test základních činností dle Barthelové by měl být využit také při doplnění diagnostického procesu v oblasti výživy. Jelikož byla zjištěna statisticky významná závislost mezi stavem výživy a stupněm závislosti, přičemž stav výživy byl horší u jedinců závislých na péči druhé osoby. Na základě výzkumu bylo zjištěno, že senioři v sociálních zařízeních disponovali větším množstvím ošetřovatelských diagnóz, zhoršenou výživou a vyšší závislostí na péči druhé osoby. Proto je důležité, aby i v této oblasti byl erudovaný ošetřovatelský personál, který bude mít dostatečné znalosti v oblasti ošetřovatelské diagnostiky a bude umět stanovit efektivní plán ošetřovatelské péče. Závěr: K identifikaci ošetřovatelských diagnóz u seniorů může být využit klasifikační systém NANDA International. Zjištěné výsledky mohou přispět k vytvoření jednotných sad ošetřovatelských dia

Abstract:

gnóz, specifické pro oblast péče o seniory, a korespondující standardizované nástroje, které povedou k objektivizaci ošetřovatelských diagnóz. Tato sada by měla také obsahovat vhodné intervence, reflektující kompetence vymezené legislativou České republiky. To by přispělo k efektivnější ošetřovatelské péči a usnadnilo práci sester při provádění ošetřovatelského procesu u seniorů.

Abstract:

Introduction: The nurse in clinical practice provides nursing care through the nursing process, which includes nursing diagnosis. The aim of the nursing diagnosis is to draw conclusions on the specific needs of the individual and to plan effective care. Due to demographic changes, is the actual to deal with standardization of diagnostic language in nursing care in older adults. Aim: The aim this thesis was to determine the frequency of selected international nursing diagnoses according to NANDA International for older adults in institutional care. Another aim was to complete the diagnostic process using standardized scales that led to the detection of nutritional status and independence of older people in institutional care. And at the same time determine the specificity and sensitivity of the used standardized scales for established nursing diagnoses. Methods: This is a quantitative research. It was created by a special record sheet, which included a selection of nursing diagnosis according to NANDA International (Class of Food intake, Activity / Exercise and Self-care) and measuring scales to complete diagnostic algorithm (MNA - Mini Nutritional Assesssment Assesmentt and BI - Barthel Index). To verify the applicability of the nursing diagnosis was used diagnostic algorithm by NANDA International. The research sample consisted of 200 elderly people in institutional care. Mean age was 76.7 years (standard deviation was 8.7). The minimum age was 60 years and the highest age was 94 years. Men were 75 (38%) and women were 125 (63%). The research was preceded by obtaining ethics committee approval, management of selected institutions and the respondents. Data were processed statistically. Statistical tests were evaluated at a significance level of 5%. Results and discussion: All research aims of the work were met. Nursing diagnoses according to NANDA International classes of (Class of Food intake, Activity / Exercise and Self-care deficit appears to be suitable for use in nursing care for the older adults, both in health and social area. Based on the diagnostic algorithm were most frequently found nursing diagnose related to malnutrition and self-care deficit. Diagnostic algorithm according to NANDA International is appropriate to supplement standardized measuring tools. Mini Nutritional Assesssment can be used for objectification Imbalanced Nutrition: Less Than Body Requirements nursing diagnosis, because this diagnose was achieved wholly sensitivity. Barthel Index is suitable for the objectification of nursing diagnoses from the class of Self-care deficit, which was achieved a great degree of sensitivity and specificity. We believe that the Barthel Index should also be used in the completion of the diagnostic process in the field of nutrition. Since there was found a statistically significant relationship between nutritional status and degree of dependence, moreover nutritional status was worse in individuals dependent on the care of another person. Based on research, it was found that older adults in social institutions dispose plenty of nursing diagnoses, impaired nutrition and increased dependence on the care of another person. Therefore, it is important that in this area was educated nursing staff, who will have sufficient knowledge in the field of nursing diagnosis and be able to establish effective nursing care plan. Conclusion: To identify nursing diagnoses can be used NANDA International classification system. The results may contribute for creation single set of nursing diagnoses that would be specific to the care of the older adults and the corresponding standardized tools that would lead to the objectification of nursing diagnoses. This single set should also include appropriate interventions that would reflect the competencies defined by the legislation of the Czech Republic. This would contribute to a more efficient nursing care and simplify nurses work in implementing the nursing process in older adults.
