Casual analysis asthma bronchiale and nasal allergy at patients cared in clinic preventive and occupational medicine

Abstract:

ABSTRAKT Úvod: Astma a respirační alergie jsou onemocnění dýchacího traktu, která jsou důsledkem alergických reakcí. V posledních letech přibývá alergických onemocnění a atopie (zvýšená hladina specifických IgE protilátek v krvi). Proto i do pracovního prostředí přichází čím dál tím více k alergii predisponovaných osob. Tento trend posledních let způsobuje potíže při posuzování zdravotní způsobilosti k práci. Klinika pracovního a preventivního lékařství ve Fakultní nemocnici Ostrava řeší tento problém tím, že provádí komplexní alergický profil zaměstnanců, kteří jsou zařazeni nebo budou zařazeni do pracovního prostředí s výskytem potenciálních alergenů. Cíl práce: Zjistit příčiny astmatu a rýmy (atopie, pozitivní alergická rodinná anamnéza a přecitlivělost na běžné alergeny v osobní anamnéze) u dvou skupin pacientů: u pacientů s přiznanou nemocí z povolání a u pacientů s neprofesionálním astmatem a rýmou. Dalším cílem je srovnání těchto dvou skupin ve výskytu zjištěných příčin. Základní hypotézou je, že není rozdíl ve výskytu sledovaných znaků mezi oběma skupinami. Materiál a metodika: 1. Vyhledat z dokumentace všechny pacienty, u nichž byl vyšetřen alergický profil v letech 2003 ? 2009 na klinice pracovního a preventivního lékařství FNSPO Ostrava a měli v osobní anamnéze astma bronchiale nebo alergickou rýmu 2. Sledované znaky (pohlaví, věk, atopie, rodinná anamnéza, přecitlivělost na běžné alergeny v osobní anamnéze, kouření, profesní expozice alergenům) zaznamenat do tabulky v programu Microsoft Excel 3. Statisticky zpracovat zjištěná data 4. Interpretovat výsledky vzhledem k příčinám astma bronchiale a alergické rýmy Výsledky: Při porovnávání výskytu sledovaných znaků u obou souborů byl zjištěn statisticky významný rozdíl u všech těchto sledovaných znaků: atopie (p = 0,00008), pozitivní alergická rodinná anamnéza (p = 0,0000), pozitivní alergická anamnéza u matky (p = 0,0005), přecitlivělost na běžné alergeny (p = 0,0000). Všechny tyto sledované znaky se mnohem více uplatňovaly u souboru pacientů s neprofesním onemocněním (n = 364). U pacientů s profesním onemocněním (n = 183) se jako příčina uplatňovaly v převaze profesní alergeny. Nejčastější etiologickou profesní noxou byla mouka (44 %). Dále byl zjištěn vyšší výskyt atopie u pacientů mladších 40 let, a to u obou skupin pacientů, kde u pacientů s neprofesním onemocněním (n = 364) se vyskytovalo 68 % atopických jedinců (p = 0,0000) v této věkové skupině a u pacientů s profesním onemocněním (n = 183) 57 % atopických jedinců (p = 0,0057). Závěr: Práce potvrdila, že na vzniku profesionálního astmatu a rýmy má převažující vliv pracovní prostředí a nevýznamně se uplatňují obecné příčiny těchto onemocnění jako je atopie, pozitivní alergická rodinná anamnéza a senzibilizace na běžné alergeny v životním prostředí. Obecné příčiny těchto onemocnění se významněji uplatňují v neprofesním onemocnění astmatem a rýmou. Z těchto důvodů jsou preventivní opatření zajišťující snížení výskytu alergenů v pracovním prostředí velmi významné. Klíčová slova: alergie, atopie, asthma bronchiale, alergická rýma

Abstract:

ABSTRACT Introduction: Asthma and respiratory allergies are disorders of respiratory tract that are consequences of allergic reactions. In recent years there has been an increase in allergic disorders and atopy (higher level of specific IgE antibodies). Thus much more people with pre-disposititon to allergies enter working positions.This trend causes troubles at rating of abilities for particular jobs. The Clinic of work and preventive medicine in Teaching hospital Ostrava solves the problem by carrying out a complex allergic profile of employees who may be working surrounded by potential allergenes. Aim of work: To find out the cause of asthma and rhinitis (atopy, positive family anamnesis and oversensitivity to usual allergens in personal anamnesis) in two groups of patients: those with admitted job-related disease, and those with non-professional asthma and rhinitis. To compare these two groups on the basis of occurence of realized reasons. The hypothesis states that there is no difference in occurence of monitored features. Material and methodology: 1. To search from documentation all the patients whose allergic profile was examined at the Clinic of work and preventive medicine in Teaching hospital Ostrava in 2003 ? 2009 and had asthmana bronchiale or allergic rhinitis in their personal anamnesis 2. To mark monitored features (age, atopy, family anamnesis, oversensitivity to usual allergens in personal anamnesis, smoking, exposition to allergens at work) in an Microsoft Excel chart 3. To work out the data statistically 4. To explain the results in respect to causes of asthma bronchiale and allergic rhinitis Results: When comparing occurence of monitored features in both groups there was found out a significant difference in all these monitored features: atopy (p = 0,00008), positive allergic family anamnesis (p = 0,0000), positive allergic anamnesis from mother?s side (p = 0,0005), oversensitivity to usual allergens (p = 0,0000). All the monitored features were applied much more with the group of patients with non-professional diseases (n = 364). At the patients with job-related diseases (n = 183) the causes were mostly job-related allergens. Flour (44%) was the most frequent job-related etiological noxa (44 %). Then there was marked a higher occurence of atopy with patients under the age of 40, with both groups of patients, where at patients with non-professional disease (n = 364) there was 68 % of atopic individuals (p = 0,0000) in this age group; and at patients with job-related disease (n = 183) there was 57 % of atopic individuals (p = 0,0057). Conclusion: The thesis confirmed that they are the working conditions that influence most significantly arising of job-related asthma nad rhinitis; general causes - as atopy , positive family anamnesis and senzibilization to usual environmental alergens - are only minor in their impact. General causes of these disorders have more significant impact on non-profesional asthma and rhinitis. These reasons show that preventive measures ensuring decrease of allergens in working place is very important. Key words: allergy, atopy, asthma bronchiale, allergic rhinitis
