The effect of kinesiotape on the static heel valgosity and dynamic parameters of the preschool-aged children's gait

Abstract:

Z pohledu fyzioterapeuta chybí dostatek vědeckých podkladů o účinnosti běžně užívaného kineziotapingu při léčbě deformit nohou u dětí. Cílem diplomové práce bylo zhodnotit vliv kineziotejpu nejen na statickou valgozitu paty u dětí předškolního věku, ale i dynamické parametry chůze z pohledu okamžitého účinku i dlouhodobějšího sledování. Se souhlasem zákonných zástupců se praktické části práce účastnilo 10 dětí (věk 5,39 ? 0,48 let) s oboustrannou valgozitou paty (9,1 ? 3,1°). Do výzkumu nebyly zahrnuty děti s vrozenými vývojovými vadami, traumatologickým poškozením dolních končetin, s neurologickým postižením, vybavené ortopedickou pomůckou a podstupující rehabilitační program. Tejpování probíhalo každý týden po dobu 3 měsíců. Byly aplikovány dva "I" tejpy pro optimalizaci valgozity paty a tonizaci musculus tibialis anterior. K objektivizaci dat byl využit goniometr a dynamický chodník h/p/cosmos?Rehawalk?. Měřenými parametry byly hodnoty statické valgozity paty, úhlové nastavení nohy během stojné fáze chůze, trajektorie pohybu CoP v rámci chodidla a silové zatížení nohy během chůze a doba kontaktu jednotlivých oddílů nohy s podložkou. Z výsledků práce vyplývá, že dlouhodobá aplikace kineziotejpu může být vhodnou metodou při korekci statické valgozity paty u dětí a zároveň přispívá k optimalizaci sledovaných parametrů chůze.

Abstract:

From the physiotherapist's point of view, there is a lack of scientific evidence on the effectiveness of the commonly used kinesiotaping in treatment of children's foot deformities. The aim of this diploma thesis was to evaluate the effect of kinesiotaping not only on static heel valgosities in preschool children, but also on dynamic gait parameters in terms of the immediate effect and long-term monitoring. A total of 10 children (aged 5.39 ? 0.48 years) with bilateral heel valgosity (9.1 ? 3.1°) participated in the practical part of the thesis with consent of their legal representatives. Children with congenital malformations, lower limb traumas, neurological impairments and those equipped with orthopedic aid or undergoing a rehabilitation program were not included in the research. Kinesiotaping therapy took place every week for 3 months. Two "I" tapes were used for the heel valgosity optimization and the anterior tibialis muscle toning. Goniometer and dynamic walkway h/p/cosmos?Rehawalk? were used for a better objectivity. The measured parameters were static values of the heel valgosity, angular foot adjustment during the stance phase of walk, CoP movement trajectory within the foot, force loading of the foot during walk and the contact time of different foot parts with the mat. The results of the research show that long-term application of the kinesiotaping may be an appropriate method for the static heel valgosity correction in children and at the same time contributes to the optimization of the monitored gait parameters.
