Potenciál variability v genech kódujících adipokiny v neurobehaviorálním řízení příjmu potravy u české obézní a neobézní populace

Variability in the adipokine genes in the neurobehavioral control of feeding in the Czech obese and non-obese population

Abstract:

Obecně se předpokládá, že specifické genetické varianty jsou podkladem specifických vzorců příjmu potravy a složení potravy, dosud bylo ovšem provedeno jen velmi málo studií sledujících asociace genotyp-fenotyp s ohledem na výživu jedince. Cílem této studie proto bylo identifikovat možné genetické varianty v jednonukleotidových polymorfismech (SNPs) v genech souvisejících s obezitou [(leptin (LEP): rs7799039, rs2167270; leptinový receptor (LEPR): rs1137101, Lys204Arg; adiponektin (ADIPOQ): rs2241766, T94G; ghrelin (GHRL): hCV25607739, rs696217; interleukin-6 (IL-6): rs1800797, rs1800795; proopiomelanokortin (POMC): rs3754860, rs1009388; receptor pro melanokortin-4 (MC4R): rs2229616 a angiotensin-konvertující enzym (rs4646994)], které mohou přispívat ke specifickému potravnímu chování nebo časové struktuře příjmu potravy. Studie byla provedena ve dvou fázích – ve fázi I byla vyhodnoceno 8 polymorfismů u skupiny 185 dobrovolníků a následně byly analýzy provedeny znovu pro 14 SNPs na souboru 409 subjektů, které byly vybrány z celkového počtu 550 jedinců ve studii. Výsledky pilotní studie neprokázaly signifikantní asociace se studovanými SNPs. Další analýza provedená na rozšířeném souboru jedinců pak poskytla řadu signifikantních asociací daných polymorfismů se sledovanými antropometrickými parametry, složením stravy, vzorci stravování a lifestylovými charakteristikami. Na základě trendů pozorovaných v těchto analýzách jsme se zaměřili zejména na polymorfismus ADIPOQ +45T/G, který má podle našeho mínění ze sledovaných SNP nejsilnější nutrigenetický potenciál. Alela ADIPOQ +45G se projevuje jako signifikantní prediktor pro příjem sacharidů i tuků ve stravě, což je zajímavé zejména s ohledem na dříve popsaný prodiabetogenní efekt této alely. Na základě získaných dat se domníváme, že alespoň část tohoto prodiabetogenního efektu by u nosičů ADIPOQ +45G alely mohla být způsobena dlouhodobou zvýšenou sacharidovou náloží ve stravě. Toto zjištění může mít potenciálně velký význam pro individuální sestavení terapie „na míru“ pacientovi nebo pro farmakogenetickou intervenci v budoucnosti.

Abstract:

Although it has been reported that genetic variations underlyling specific eating patterns and specific food preferences are considered to represent a significant risk factor for obesity, only a few reports have focused on the genotype-by-nutrient associations so far. The aim of the thesis was to identify possible genetic variants in common SNPs [(leptin (LEP): rs7799039, rs2167270; leptin receptor (LEPR): rs1137101, Lys204Arg; adiponectin (ADIPOQ): rs2241766, T94G; ghrelin (GHRL): hCV25607739, rs696217; interleukin-6 (IL-6): rs1800797, rs1800795; proopiomelanocortin (POMC): rs3754860, rs1009388; melanocortin-4 receptor (MC4R): rs2229616 and angiotensin-convertin enzyme (rs 4646994)] that contribute to specific eating behaviour in the context of specific obesity-related anthropometric features. The thesis was structured into three specific subanalyses – the preliminary results were obtained on the subgroup of 185 volunteers and subsequently, the genotype data were reanalyzed on the total of 409 study subjects in relation to nutritional characteristics. The preliminary nutritional results of the pilot study did not reveal any major associations with the investigated SNPs. The data reanalysis performed on the 409 study subjects yielded numerous significant associations of the investigated SNPs with the anthropometric features as well as dietary and lifestyle characteristics of the individuals Based on the results, we focused on ADIPOQ +45T/G polymorphism and further analyzed the food records of the study subjects with regards to the known prodiabetogenic effect of the polymorphism, whereas the G allele was associated with the increased daily intake of carbohydrates and therefore, we suggest that a proportion of the prodiabetogenic effect of the polymorphism might be due to its influence on eating behaviour. This might have potentially important impliacations for individual therapy-tailoring or pharmacogenetic approaches in the future.
