Factors influencing the accuracy of dietary intake data of energy and micronutrients in 24hour recall and record methods

Abstract:

Úvod: Výběr metody pro zjišťování výživové spotřeby může významně ovlivnit výsledky. Jedním z hlavních zdrojů chyb při nutričním hodnocení je tzv. misreporting – tedy zkreslení uváděných informací o příjmu respondentem, a to jak ve smyslu jeho nadhodnocení – overreporting, tak podhodnocení – underreporting. Misreporting představuje vážnou chybu v odhadu přívodu energie, ale i jednotlivých nutrientů. Určit tyto chyby při zjišťování výživové spotřeby není jednoduché, ale jejich existence vytváří otázku, zda a jak interpretovat a používat získaná data. Následně tak mohou být skryty nebo zkresleny vztahy mezi výživovou spotřebou a s výživou spojenými onemocněními. Před rozhodnutím, zda vyloučit data ovlivněná misreportingem, je nezbytné zjistit více o tom, zda je podhodnocování energetického přívodu náhodným jevem v populaci, koho postihuje a jaké chyby z něj vyplývají. Dizertační práce má za cíl shrnout a usnadnit porozumění problému misreportingu popsáním chyb při nutričním hodnocení, které vedou k under- nebo overreportingu, pojmenovává determinanty těchto chyb a metody jejich identifikace a posouzení rozsahu. Práce poskytuje informace, které mohou být použity k minimalizaci těchto chyb, a poukazuje na další možnosti výzkumu v této oblasti. Práce se zaměřuje na chyby při použití dvou obvyklých metod zjišťování průměrného příjmu energie nebo nutrietů v populaci: 24hodinový recall a záznam stravy. Metody: Práce plnila úkol evropského projektu EURRECA (Europian micronutrient recommendations aligned), jehož cílem bylo sjednotit v Evropě tvorbu nutričních doporučení pro mikronutrienty. Jeden z dílčích úkolů projektu byl prostudovat problematiku chyb při zjišťování výživové spotřeby, konktrétně podhodnocování a nadhodnocování množství přijaté stravy a tím i energie a nutrientů. Metodou systematického review byla prohledána dostupná literatura a studie věnujícící se problematice misreportingu a ty studie, které identifikují osoby zkreslující záznam stravy nebo 24hodinový recall nebo přímo míru zkreslení přívodu energie a některých mikronutrientů. Posouzením kvality studií a zařazením do výběru na základě předem definovaných kritérií bylo identifikováno 37 studií, jejichž data byla extrahována a analyzována. Výsledky: Studie vybrané do systematického přehledu používaly nejednotné metody pro identifikaci misreportingu, případně vyčíslení jeho míry. Nejčastější metodou byla metoda Goldbergova cut-off, která stanovuje konfidenční limity pro označení respondentů jako pod- nebo nadhodnocující. Při zjišťování přívodu energie u dospělých metodou 24hodinového recallu a záznamu stravy s pomocí odhadu nebo vážení dochází cca u 30 % respondentů k podhodnocování přívodu energie, a to asi o 7–20 %. Zároveň se objevuje i nadhodnocování přívodu energie, a to u 1–7 % respondentů, přičemž je uváděný přívod nadhodnocen o 7–13 %. Nebyl zjišťěn signifikantní rozdíl v míře podhodnocování mezi jednotlivými metodami (24hodinový recall, záznam stravy s pomocí vážení a odhadu). Podobný trend byl sledován i u mikronutrientů – konkrétně u železa, vápníku a vitaminu C, kdy u mužů i žen bylo zjištěno signifikantní podhodnocení hodnot přívodu těchto nutrientů u osob, které zároveň podhodnocovaly přívod energie oproti těm, které přívod energie nepodhodnocovaly. Závěr: Zkreslování uváděného příjmu stravy je závažný problém a je žádoucí mu věnovat pozornost při zjišťování přívodu energie a nutrientů. Abstract:

Introduction: The choice of the type of assessment method may influence the results. One of the main sources of error in dietary assessment is misreporting, comprising both under- and overreporting. Misreporting represents a severe error in the estimation of energy intake (EI), but also in that of other nutrients. It is difficult to establish misreporting but even when it has been identified, it is unclear whether or how these data may be interpreted and used. Relationships between dietary intakes and diet-related diseases could consequently be obscured or confounded. Before deciding whether to exclude data affected by misreporting it is necessary to know more about whether low-energy reporting is a random event in the population, who it affects and any bias resulting from it. The doctoral thesis is trying to summarize and facilitate a better understanding of the problem of misreporting by describing measurement errors in dietary assessment resulting in under- or overreporting, to find out their determinants and methods used to identify misreporting, and to judge the magnitude of these errors. The thesis will also provide information that may be used to minimize these errors, to highlight gaps in our knowledge, and to recommend future priorities for research. The thesis focuses on 24hour recalls and food records used to assess average intakes of a population. Methods: The thesis followed the task of the European project EURRECA (Europian micronutrient recommendations aligned), aiming at harmonising nutritional recommendations for micronutrients in Europe. One of the partial tasks of this project was to study the measurement error in nutritional assessment, specifically underreporting and overreporting of food intake and thus also of energy and nutrients. The research included a systematic review of available literature and studies dealing with misreporting and identifying the respondents who misreported the food record or a 24hour recall or directly the magnitude of energy and micronutrient misreporting. By assessing the quality of the studies and including them into the final selection based on predefined criteria we identified 37 studies, the data of which were extracted and analysed. Results: The studies chosen for the review did not use the same methods for misreporting identification or quantifying its magnitude. The most used method was Goldberg cut-off which sets the confidence limits for identifying the respondents as under- or overreporters. While assessing the energy intake in adults using the 24hour recall and food record (estimated or weighed) about 30 % respondents tend to underestimate the energy intake by about 7–20 %. Overestimating the energy intake occurs in 1–7 % respondents and the intake is overestimated by 7–13 %. No significant difference in the magnitude of underreporting between different methods (24hour recall, weighed or estimated food record) was found. A similar trend was observed in micronutrients – specifically in iron, calcium and vitamin C. In both men and women there was a significant underreporting of the intake of these nutrients in respondents who also underreported the energy intake in contrast to those who did not. Conclusion: Misreporting of food intake is a serious problem and it is desirable to give it enough attention when assessing the intake of energy and nutrients. Using the data without any correction could lead to false conclusions of studies, to strengthening or weakening of some relations between energy or nutrient intake and health indicators and provide false values for nutritional recommendations.
