Bone mineral density in patiens with celiac disease

Abstract:

Celosvětový výskyt celiakie a osteoporózy v posledních letech významně stoupá. Osteoporóza je dnes považována za civilizační chorobu. A význam těchto nemocí není zanedbatelný jak z hlediska klinického tak i veřejného zdravotnictví. Celiakie je nemoc velmi dobře popsaná a známá, ale stále se na ni málo myslí (nediagnostikované formy celiakie tvoří celosvětově 70 až 80 %). Teoretická část této práce je zaměřena na informace jak o celiakii, tak i o osteoporóze, jejich epidemiologické situaci, patogenezi, diagnostice, vyšetřovacích metodách a možnostech prevence a léčby. Dále pak popisuje klasifikaci, rizikové faktory a klinický obraz. Poslední kapitola je věnována problematice konkrétních aspektů poruch kostní hmoty při celiakii. Pro práci je použita retrospektivní studie, vyšetřovaná skupina pacientů a výsledky denzitometrického vyšetření pocházejí z osteologického centra. O spojení celiakie s osteoporózou existuje dostatek dat a údajů, byť se číselná vyjádření liší. Úkolem praktické části tedy bylo zpracování a statistické vyhodnocení hodnot a zjištění dané závislosti obou nemocí u konkrétní skupiny pacientů. Z výsledků vyplývá důvodná potřeba screeningového vyšetření osteoporotických změn u pacientů s diagnostikovanou celiakií.

Abstract:

The worldwide incidence of celiac disease and osteoporosis has been on a sharp rise in the past years. Osteoporosis is now considered as a disease of civilization. Moreover, the importance of these diseases cannot be neglected from both the clinical and public health point of view. Celiac disease is well-known and very well described, however, it is still beyond our focus. (undiagnosed forms of celiac disease account for 70 to 80 % worldwide). The theoretical part of this work focuses on both the information about celiac disease and osteoporosis, their epidemiological situation, pathogenesis, diagnosis, examinational methods and possibilities of prevention and treatment. It further describes classification, risk factors and a clinical picture. The last chapter deals with the issues of particular aspects of bone disorders in celiac disease. The work uses the retrospective study. The examined group of patients and the densitometry results come from the osteologic centre. There is a lot of information and data on the celiac disease and osteoporosis connection even though the figures differ. The practical part therefore was to process and statistically evaluate the findings and to determine the dependence of both diseases in a particular group of patients. The results show that there is a significant need for screening examination of osteoporotic changes in patients diagnosed with celiac disease.
