Body temperature of patients during general anaesthesia

Abstract:

Diplomová práce je zaměřena na nežádoucí perioperační hypotermii během operačního výkonu v celkové anestezii. Pojem perioperační hypotermie je i dnes poněkud opomíjený. Pokles teploty tělesného jádra pod 36 °C během operačního výkonu je definován jako perioperační hypotermie. Cílem zdravotnického personálu je nejen udržení tělesné teploty pacienta v optimálním rozmezí, ale i případné řešení perioperační hypotermie. V důsledku prodělané perioperační hypotermie se objevuje nárůst perioperačních komplikací a následně se prodlužuje délka hospitalizace. V druhé části diplomové práce jsou popsány cíle práce. První cíl si klade za úkol zjistit, v jakém počtu a v jaké závažnosti se perioperační hypotermie u pacientů vyskytuje. Zjišťujeme, zda existuje závislost mezi vznikem hypotermie a použitím pomůcek k prevenci perioperační hypotermie. Druhý cíl se zabývá prevencí perioperační hypotermie. Zjišťujeme, zda délka výkonu ovlivňuje pooperační úpravu tělesné teploty do fyziologického rozmezí. Zda existuje závislost mezi typem operačního výkonu a závažností hypotermie. Posledním cílem je vytvořit soubor edukačních doporučení pro praxi. Vedle těchto cílů bylo stanoveno celkem 6 hypotéz. Do studie bylo zahrnuto 168 pacientů, ve věku 20–59 let, podstupujících operační výkon v celkové anestezii trvající déle než 90 minut. Metoda sběru dat probíhala formou zápisu do záznamového archu. Teplota byla pacientům měřena celkem jedenáctkrát, v předem určených časových odstupech. Získaná data byla kvantitativně zpracována a interpretována převážně formou krabicových grafů a kontingenčních tabulek. Dosažené výsledky výzkumného šetření byly následně srovnány se současnými doporučeními National institute for health and care excellent, American Society of PeriAnesthesia Nurses, Association of Operating Room Nurses a dalšími studiemi, které se daným tématem zabývaly. Na základě uvedeného porovnání byl vytvořen soubor edukačních doporučení pro zdravotnický personál operačních sálů, především pro anesteziologické sestry.

Abstract:

The diploma thesis is focused on undesired perioperative hypothermia during surgery in general anesthesia. The term perioperative hypothermia is still somewhat neglected today. The decrease in body core temperature below 36 ° C during surgery is defined as perioperative hypothermia. The sekond objektive is to deal with the preventiv of perioperative hypotermia. As a result of the perioperative hypothermia, there is an increase in perioperative complications and consequently the length of hospitalization is prolonged. The second part of the diploma thesis describes the aims of the thesis. The first objective is to find out in what number and severity the perioperative hypothermia in patients occurs. We determine whether there is a dependency between the occurrence of hypothermia and the use of devices to prevent perioperative hypothermia. In the second aim we are dealing with the prevention of perioperative hypothermia. We determine whether the duration of the exercise affects the postoperative adjustment of the body temperature to the physiological range. Whether there is a dependency between the type of surgery and the severity of hypothermia. The final a set of educational recommendations for practice. A total of 6 hypotheses were set for these goals. The study involved 168 patients, aged 20-59 years, undergoing surgery in total anesthesia lasting over 90 minutes. The data collection method took the form of writing to the record sheet. The temperature was measured in patients eleven times, at predetermined time intervals. The data obtained was quantitatively processed and interpreted mainly in the form of box charts and contingency tables. The results of the research were subsequently compared with the current recommendations of the National Institute of Health and Care, the American Society of Periesthesia Nurses, the Association of Operational Nurses and other studies dealing with the topic. Based on this comparison, a set of educational recommendations for medical staff in operating theaters was developed, especially for anesthetist nurses.
