Changes in executive cognitive function in adolescents with type 1 diabetes

Abstract:

Cílem práce je zjistit, nakolik onemocnění diabetes mellitus 1. typu (dále jen T1D) ovlivňuje u dospívajících jedinců jejich exekutivní funkce. Práce se snaží o komplexní uchopení tématu. V disertační práci byly použity rozdílné pohledy na vztah mezi onemocněním T1D a exekutivními funkcemi dospívajících: pohled dospívajících jedinců, pohled jejich rodičů a pohled profesionálů (lékaře a psycholožky). Dospívající s T1D ve věku 11-18 let absolvovali psychologické testy exekutivních funkcí (n=53) a neurofyziologické vyšetření elektrické mozkové aktivity (n=22). Výsledky byly porovnány s výsledky kontrolní skupiny vrstevníků bez diabetu (n=37, n=19). Pomocí dotazníku IPQ-R-CZ byly zmapovány názory dospívajících na svoji nemoc (n=53). Rodiče zhodnotili exekutivní funkce svých dětí s T1D pomocí dotazníku BRIEF (n=41). Součástí práce je 8 kazuistik diabetiků, které kombinují výsledky lékařských vyšetření, lékařské zkušenosti s léčbou diabetu u dospívajících s výsledky psychologických vyšetření. Provedený výzkum neprokázal v našem výzkumném souboru statisticky významné narušení exekutivních funkcí dospívajících s T1D, s výjimkou rodiči vnímaného zhoršení v oblasti přesunu pozornosti, emoční kontroly, plánování a organizace, iniciativy a kontroly chování. Z našeho šetření rovněž vyplynulo, že dospívající diabetici jsou dobře informováni, uvědomují si vážnost svého onemocnění a učí se s nemocí žít. Přesto nezanedbatelná část diabetiků přiznává depresivní náladu (32 %), obavy (43 %), ba i vztek (24 %) prožívané směrem k nemoci. Významný vliv na kolísání glykemií má aktuálně prožívané vývojové období - dospívání - kdy diabetici přebírají zodpovědnost za péči o své onemocnění a hledají svoji identitu a místo v životě. Ukázalo se také, že při léčbě diabetu u dospívajících je správné věnovat pozornost psychologickým aspektům zdravotního chování dospívajících diabetiků a do péče standardně zapojovat klinické psychology.

Abstract:

The aim of the dissertation is to find how much does type 1 diabetes mellitus (T1D) influence executive functions in adolescents. The dissertation strives to examine the given topic comprehensively. In the dissertation, various views on the relationship between T1D and executive functions in adolescents are presented: a view provided by adolescents themselves, by their parents, and by professionals (a physician and a psychologist). T1D adolescents aged 11-18 underwent psychological testing of executive functions (n=53) and neurophysiological examination of electrical activity of the brain (n=22). The results were compared to the results of a control group consisting of non-T1D peers (n=37, n=19). The IPQ-R-CZ questionnaire was used to map the adolescents' opinion on their disease (n=53). Parents assessed the executive functions of their children suffering from T1D by means of the BRIEF questionnaire (n=41). The dissertation includes 8 case reports of diabetic patients, combining the results of medical examinations, medical experience with diabetes treatment in adolescents, and results of psychological examinations. The research of our set of patients did not reveal any statistically significant impairment of executive functions of adolescents with T1D, except for impairment of attention shifting, emotional control, planning and organization, initiative and behaviour control perceived by the adolescents' parents. Our research has also found that adolescent diabetics are well informed; they are aware of the severity of their disease and are learning how to live with it. Nevertheless, non-negligible proportion of the diabetics admits depressive mood (32%), fear (43%), and even anger (24%) in relation to the disease. Glycaemia fluctuation is influenced significantly by the actual development period - adolescence - when diabetics assume responsibility for care of their disease and look for their identity and their place in life. It has also been shown that attention should be paid to psychological aspects of health-related behaviour of adolescent diabetics and clinical psychologists should be involved in care as a standard.
