The effect of intratumoural heterogeneity on the response to therapy in patients with colorectal carcinoma

Abstract:

Current methods of relapse prediction in stage 2 colorectal cancer do not take into account the intratumoural heterogeneity that plays an important role in the development of this disease. Therefore, in this thesis, I focused on the region of the invasive front in predicting relapse, which is important for tumour spread and metastasis formation, but has not yet been investigated in this context. The aim of this work was to investigate whether gene expression of the invasive front is a better predictor than whole tumour expression in predicting relapse. Based on transcriptomic data from oligonucleotide microarrays, I used penalized logistic regression to create classifiers dividing patients into early relapse and no relapse groups. One classifier was trained on samples from the whole tumour. The other on samples from the invasive front. The results suggest that the invasive front has a unique expression profile that is distinct from the rest of the tumour and contributes significantly to the correct prediction of relapse. Thus, gene expression of this specific region could be useful in further research. But it is necessary to first validate the created classifiers on an independent dataset.

Abstract:

Aktuálne metódy predikcie relapsu kolorektálneho karcinómu v štádiu 2 nezohľadňujú vnútronádorovú heterogenitu, ktorá hrá pri rozvoji tohto ochorenia významnú úlohu. V tejto práci som sa preto zamerala pri predpovedi relapsu na oblasť invazívneho frontu, ktorá je dôležitá pre šírenie nádoru a tvorbu metastáz, ale nebola v tomto kontexte zatiaľ skúmaná. Cieľom tejto práce bolo zistiť či je génová expresia invazívneho frontu pri predpovedi relapsu lepším prediktorom ako expresia celého nádoru. Na základe transkriptomických dát z oligonukleotidových mikročipov som pomocou penalizovanej logistickej regresie vytvorila klasifikátory rozdeľujúce pacientov do skupín so skorým relapsom a bez relapsu. Jeden klasifikátor bol trénovaný na vzorkách z celého nádoru, druhý na vzorkách z invazívneho frontu. Výsledky naznačujú, že invazívny front má jedinečný expresný profil, ktorý sa odlišuje od zvyšku nádoru a významne prispieva k správnej predikcii relapsu. Génová expresia tejto špecifickej oblasti by teda mohla byť užitočná pri ďalšom výskume. Najprv je však nutné vzniknuté klasifikátory validovať na nezávislom dátovom súbore.
