Examination of platelet aggregations on the Multiplate device

Abstract:

ABSTRAKT Úvod: Krevní destičky se podílejí na hemostáze tvorbou primární hemostatické zátky a aktivací koagulačních faktorů. Pro tento děj je důležitý jednak jejich správný počet (kvantita), ale také jejich funkčnost (kvalita). K vyšetření agregací krevních destiček se používá optická či impedanční agregometrie. Cíle práce: Cílem této bakalářské práce bylo vyšetřit výkonnostní charakteristiky, preanalytickou fázi a vliv Covid-19 na funkčnost krevních destiček měřených na přístroji Multiplate. Metodika: Pro vyšetření agregací krevních destiček byl prováděn ADPtest, COLtest, ASPItest a TRAPtest na přístroji Multiplate. Počet krevních destiček byl vyšetřen na hematologickém analyzátoru Sysmex XN 9000. Vzorky byly odebírány do zkumavek značky Sarstedt s K3EDTA, hirudinem, případně 3,2% citrátem sodným. Celkem bylo vyhodnoceno 9 zdravých dárců a 56 pacientů v rámci různých cílů. Výsledky: Byl zjištěn statisticky významný rozdíl mezi transportem donáškou a potrubní poštou (p = 0,004). Mezi vyšetřením z hirudinové a citrátové krve nebyl zjištěn rozdíl v interpretaci výsledků. Při měření stability byl zjištěn statisticky významný rozdíl po 3 hodinách od odběru vzorku. U zemřelých pacientů s těžkým průběhem Covid-19 vyšly mediány parametrů agregace 24 U (ADPtest), 44 U (COLtest), 40 U (ASPItest) a 47 U (TRAPtest). Závěr: Vedle optické agregometrie lze provést vyšetření agregací na přístroji Multiplate impedanční metodou. Vyšetření se provádí z plné krve. Je nutné dodržovat preanalytickou fázi. Metodu limituje počet krevních destiček (min. 100 x 109 /L). Přístroj Multiplate se s výhodou používá k měření agregací u pacientů s těžkým průběhem Covid-19.

Abstract:

ABSTRACT Introduction: Platelets are involved in hemostasis by forming a primary hemostatic clot and activating coagulation factors. Their correct number (quantity), but also their functionality (quality) is important for this process. Optical or impedance aggregometry is used for examination of platelet aggregations. Aims of the work: The aim of this bachelor thesis was to investigate the performance characteristics, pre-analytical phase and the effect of Covid-19 on the functionality of platelets measured on a Multiplate device. Methods: ADPtest, COLtest, ASPItest and TRAPtest were performed on a Multiplate device to examine platelet aggregation. Platelet counts were examined on a Sysmex XN 9000. Blood was taken in Sarstedt tubes with K3EDTA, hirudin, or 3.2% sodium citrate. A total of 9 healthy donors and 56 patients were measured within different aims. Results: A statistically significant difference was found between transport by delivery and pneumatic tube system (p = 0.004). There was no difference in the interpretation of the results between the hirudin and citrate blood tests. Statistically significant difference was found when measuring stability 3 hours after sample collection. In deceased patients with severe Covid-19, the median parameters of aggregation were 24 U (ADPtest), 44 U (COLtest), 40 U (ASPItest) and 47 U (TRAPtest). Conclusion: In addition to optical aggregometry, it is possible to perform aggregation examinations on the Multiplate impedance method. The test is performed in whole blood. It is necessary to follow the pre-analytical phase. The method is limited by the number of platelets (min. 100 x 10^9/L). The Multiplate is preferably used to measure aggregations in patients with severe Covid-19.
