Comparison of visual acuity, refraction and higher-order aberrations after standard and wavefront-guided myopic femtosecond LASIK

Abstract:

Disertační práce se zabývá hodnocením pooperačních výsledků laserových refrakčních zákroků myopických očí metodou Femto-LASIK při použití standardní a wavefront-guided ablace. Byla analyzována zraková ostrost, subjektivní refrakce a aberace vyšších řádů (HOAs). Srovnávány byly předoperační hodnoty a výsledky zákroku v 1, 3 a 12 měsících po operaci. Zákrok byl proveden za asistence femtosekundového laseru LDV Z6 (Ziemer) a excimerového laseru MEL 80 (Meditec, Carl Zeiss). Výsledky studie prokázaly výborné pooperační výsledky při použití obou ablačních profilů. Sférický ekvivalent manifestní refrakce se v jednom měsíci po operaci pohyboval pro nízkou a střední myopii v hodnotách do ±0,25D, v případě vysoké myopie do ±0,5D. Index bezpečnosti i efektivity byl roven 1.0. Myopický Femto-LASIK mírně zvyšuje střední kvadratickou hodnotu aberací vyšších řádů (RMS HOAs), zejména indukcí sférické aberace. Nepotvrdil se přínos wavefront-guided ablace v nižší indukci významných aberací vyšších řádů.

Abstract:

This thesis deals with the evaluation of postoperative results of laser refractive surgery myopic eyes by femtosecond LASIK using standard and wavefront-guided ablation. Visual acuity, subjective refraction and higher order aberrations (HOAs) was analyzed. Preoperative values and the results of surgery at 1, 3 and 12 months after surgery were compared . The operation was carried out with the assistance of femtosecond laser LDV Z6 (Ziemer) and MEL 80 excimer laser (Meditec, Carl Zeiss). The study results showed excellent postoperative results using both ablation profiles. There has been no eye with a postoperative loss of an optotype line. A spherical equivalent of manifest refraction oscillated, in the first postoperative month for low and medium myopia, within values up to ±0.25D, with high myopia up to ±0.5D. The safety and efficacy index of refractive procedure was equal to 1.0. Myopic femtosecond LASIK slightly increases the RMS of higher order aberrations (HOAs RMS), particularly induction of spherical aberration. The benefit of wavefront-guided ablation in a lower induction of significant higher-order aberrations has not been confirmed.
