Long-term performance indicators of breast cancer screening programme

Abstract:

Tato bakalářská práce se zaměřuje na faktory ovlivňující opakovanou účast v programu screeningu karcinomu prsu. V teoretické části byl objasněn pojem screening a mamografický screening, dále zde byly zahrnuty informace o sběru dat, validitě screeningových testů, indikátorech kvality programu a také informace o faktorech, které mohou ovlivňovat opakovanou účast žen v tomto programu. V rámci praktické části byla použita anonymizovaná data pocházející z Registru screeningu karcinomu prsu, který je provozován Institutem biostatistiky a analýz Lékařské fakulty MU v Brně. Cílem práce bylo implementovat vhodnou metodu na datový soubor a odhalit tak faktory, které ovlivňují opakovanou účast na mamografickém vyšetření. Následně bylo úkolem porovnat screeningová centra. Za tímto účelem byla na data v softwaru SPSS Statistics 25 použita metoda Kaplan-Meierova odhadu přežití. Statisticky významné rozdíly v absenci mamografického vyšetření v závislosti na různých proměnných byly dále vyhodnoceny pomocí log-rank testu. Screeningová centra byla porovnána na základě tříleté pravděpodobnosti absence screeningového vyšetření. Bylo prokázáno, že zejména věk žen a pořadí screeningového vyšetření při indexové mamografii ovlivňuje účast na mamografickém vyšetření.

Abstract:

This bachelor thesis aims at the factors influencing the repeated participation in a breast cancer screening program. The theoretical part clarified the concept of screening and mammographic screening, it also included information on data collection, validity of screening tests, indicators of program quality, and also information on factors that may affect the repeated participation of women in this program. In the practical part, there were used the anonymized data from the breast cancer screening register, which is operated by the Institute of Biostatistics and Analyzes of Fakulty of Medicine in Brno (IBA MU). The aim of my thesis was to implement a suitable method for the set of data, and thus reveal the factors which affect the repeated participation in mammography. The subsequent task was to compare screening centers. For this purpose, the Kaplan-Meier survival estimation method was applied to the data in SPSS Statistics 25 software. Statistically significant differences in the absence of mammography examination depending on various variables were further evaluated using the log-rank test. Screening centers were compared on the basis of a three-year probability of absence of screening examination. It has been shown that especially the age of women and the order of screening examination in index mammography influence participation in mammography examination.
