Use of control charts in performance monitoring of medical screening

Abstract:

Tato bakalářská práce se věnuje monitoringu kvality zdravotního screeningu. Screeningový program karcinomu prsu pomáhá snižovat mortalitu tohoto onemocnění. K průběžnému monitorování kvality screeningového programu byly vyvinuty indikátory kvality. Jsou to odhady kvality screeningového testu nebo screeningového procesu, senzitivita testu je zastoupena detekční mírou a specificitu testu zastupuje podíl doplňujících vyšetření. Pro monitoring kvality se začínají čím dál častěji používat metody statistické regulace procesu (SPC), především klasické Shewhartovy regulační diagramy, a dále speciální typy diagramů, jako je metoda kumulovaných součtů nebo metoda exponenciálně váženého klouzavého průměru. Indikátory kvality jsou zde jako regulované veličiny, které umožní sledovat průběh celého procesu. Výsledkem je informace o stabilitě procesu, i o jeho dynamickém vývoji. V práci je shrnuta metodika regulačních diagramů, která je následně aplikována pro monitorování kvality mamografického screeningu tří screeningových center. Shewhartův regulační diagram nebyl dostatečně citlivý pro zachycení malých změn v procesu, za vhodnější nástroj je považována metoda kumulovaných součtů a metoda exponenciálně váženého klouzavého průměru. Na konci práce jsou popsána pozitiva a negativa těchto metod a je zde zmíněno jejich další využití. SPC metody se ukázaly jako vhodné nástroje, neboť umožňují rychlou detekci potenciálních problémů v kvalitě poskytovaných vyšetření.

Abstract:

A performance monitoring of a medical screening is studied in this thesis. A mortality of breast cancer could be lowered by a screening programme. Performance indicators were developed for continuous performance monitoring of the screening programme. These are estimates of screening test or screening process performance; sensitivity is represented by the detection rate, and specificity is represented by the further assessment rate. Statistical process control (SPC) methods, especially classical Shewhart control charts and special types of diagrams as cumulative sum method or exponentially weighted moving average method, are more commonly used for performance monitoring. Performance indicators stand as controlled quantities that allow us to observe execution of a process. SPC provide information about stability of the process and its dynamic development. In this thesis, the methodology of control charts is summarized, which is subsequently applied for performance monitoring of mammography screening at three screening centres. Shewhart control chart has not been sufficiently sensitive for detection of small changes in the process; cumulative sum method and exponentially weighted moving average method are considered as a more appropriate tool. The positives and negatives of these methods are described at the end of the thesis, as well as a comment on their further use. SPC methods have been proven as efficient tools as they provide fast detection of potential problems in performance of provided investigations.
