Physiotherapy after injuries of the ankle joint

Abstract:

Úvod: Práce se zabývá problematikou fyzioterapie po fraktuře hlezenního kloubu. Tato zlomenina je velmi častá a rekonvalescence po ní náročná, jelikož jejím následkem většinou nastávají změny i v dalekém okolí hlezenního kloubu. Např. svalové dystrofie. Cíl práce: Přispět k řešení problematiky fyzioterapie po fraktuře hlezenního kloubu v období ambulantní péče Metodika: První den našich společných terapií jsem pacienta vyšetřil a po vstupním kineziologickém rozboru jsem sestavil krátkodobý fyzioterapeutický plán. Vzhledem k povolené zátěži pacienta jsme v první fázi využili cévní gymnastiku, izometrické cvičení, techniky měkkých tkání, míčovou facilitaci. Postupně jsme zvyšovali zátěž a přidávali fyzioterapeutické koncepty, jako jsou senzomotorické stimulace, Brűgger koncept, proprioceptivní neuromuskulární facilitace, K Spiraldynamik? a Propriofoot?. Při poslední terapii jsem provedl výstupní kineziologický rozbor a následně sestavil dlouhodobý fyzioterapeutický plán. Výsledky: Po výstupním kineziologickém rozboru jsem zjistil zlepšení rozsahu pohybu v hlezenním kloubu natolik, aby proband měl správný stereotyp chůze (DF o 15° a PF o 20°). Zmenšil se otok a došlo k nárůstu svalové síly na celé operované dolní končetině. Závěr: V teoretické části je popsán hlezenní kloub, jeho funkce. Dále jsou popsány typy zlomenin a možnosti léčby. Následující část se věnuje teoretické fyzioterapii, kde jsou popsány jednotlivá vyšetření a samozřejmě samotné typy terapií. Práce pokračuje praktickou částí, kde je většina teoretické části uplatněna v praxi. V závěru je zhodnocena práce s probandem.

Abstract:

Introduction: This work deals with the problems of physiotherapy after ankle fractures. This fracture is very common and challenging mainly because the changes in the surroundings of the ankle joint. E.g. muscular dystrophy. Purpose: The purpose of my thesis is to contribute to solving the physiotherapy problems of ankle fractures during outpatient care. Methodology: On the first day of our therapy, patients are examined, and after the initial kinesiological analysis, a short-term physiotherapeutical plan is prepared. Depending on the patient's doctor's authorized load, we used vascular gymnastics, isometric exercises, soft tissue techniques, and foam-ball facilitation. Gradually, the load was increased and accordingly physiotherapeutic concepts such as sensomotoric stimulation, Brűgger concept, proprioceptive neuromuscular facilitation, Spiraldynamic? and Propriofoot? concepts were added. During the last therapy, an out patient kinesiological analysis was performed, and a long-term physiotherapeutic plan was then compiled. Results: After the output kinesiological analysis, improvement was found in the extent of the movement in the ankle so that the proband had the correct gait (DF by 15 ° and PF by 20 °). Swelling has decreased and there was an increase in muscle strength across the entire lower limb. Conclusion: In the theoretical part, it is described the ankle joint, its function. Further, the description of the types of fractures and treatment options. The following part deals with theoretical physiotherapy, where are described the individual examinations and the types of therapies used. The work continues with the practical part where most of the theoretical part is applied in practice. At the end, the work with the patient its evaluated.
