Physiotherapy treatment in arthrosis of lower extremity joints

Abstract:

Úvod: Téma své práce jsem si vybral z důvodu, že se jedná o ortopedickou problematiku a dále proto, že postihuje velké spektrum lidí od starších lidí po sportovce. Cíl práce: Cílem mé bakalářské práce je přispět k řešení problematiky fyzioterapie u vybraného klienta při artróze kolenního kloubu volbou optimálních fyzioterapeutických postupů, metod a konceptů. Metodika: Ve své práci jsem použil metody rozhovoru, pozorování, testování a měření. Zvolil jsem klientku s počínajícím III. stupněm gonartrózy na pravé dolní končetině. Na základě kineziologického rozboru, který obsahoval vyšetření aspekcí, palpací, funkční a dynamické vyšetření jsem si stanovil krátkodobý plán. Během terapie jsem v polovině intervence (při 6.terapii) provedl kontrolní měření a po ukončení terapie jsem s ohledem na výsledky stanovil dlouhodobý plán. Během práce s klientkou jsem využil mnoho metod a konceptů např. techniky měkkých tkání, izometrickou kontrakci, postfacilitační strečink, DNS, PNF, ACT?, Senzomotorickou stimulaci i pomůcky jako je thera-band, overball, fyzioball. Výsledky: Během terapie se mi podařilo odstranit otok kolem kolenního kloubu. Povedlo se mi zlepšit hybnost pately, upravit svalové dysbalance. Rozsah pohybu v kyčelním kloubu se zvýšil na obou DKK do flexe i vnitřní rotace o 5°, abdukce o 10° na PDK a 5° na LDK. V kolenním kloubu se zvýšil hybnost do flexe o 10°. Na obou končetinách vzrostla svalová síla do abdukce v kyčelním kloubu o 1 stupeň, svalová síla se zlepšila do flexe v kolenním kloubu na obou stranách o 1 stupeň, na PDK se zlepšila svalová síla do extenze o 1 stupeň. Dále se mi podařilo zlepšit aktivaci bránice a stereotyp chůze. Závěr: Během spolupráce s klientkou se nám povedlo dosáhnout cílů, které jsme si před terapií stanovili. U klientky sem projevil zájem dozvědět se víc o onemocnění a zlepšit její fyzickou kondici a psychickou pohodu.

Abstract:

Introduction: I chose the topic of my bachelor thesis for many reasons. First of all, it deals with issues in orthopaedics field of medicine and also affects a large number of people, such as the elderly and athletes. The aim: The aim of my bachelor thesis is to contribute to the solution of physiotherapy. It helps to choose optimal physiotherapeutic procedures, methods and concepts in a selected client with arthrosis of the knee joint. Methodology: Methodology I chose for my thesis are interview, observation, testing and measurement. I selected a client at beginning of III. degree of osteoarthrosis on the right lower limb. On the basis of kinesiological analysis, which includes an examination of the aspection, palpation examination, function and dynamic tests, I set short-term plan of the therapy. I performed the mid-term control analysis during the 6th therapy. At the end of the therapy I made a final kinesiological analysis to set long term plan of the recovery. I used lot of methods, concepts f.e. soft tissue techniques, postfacilitatory streching, DNS, PNF, sensomotoric stimulation and also I used thera-band, overball, fyzioball. Results: During the therapy I succeeded to remove a swelling around the knee. I achieved improvements movement of patella, modify muscle imbalances, increase range of movement in the hip joint on both lower limbs to flexion by 5°, interior rotation by 5°, abduction by 10° on right lower limb and the range of movement raised of 5° on left lower limb. The range of movement in knee joint on both lower limbs increased in flexion by 10°. On both lower limbs increased muscle strenght in abduction in the hip joint by 1 degree. On both lower limbs increased muscle strength in the knee flexion by 1 degree and on right lower limbs increased muscle strength in extension by 1 degree. I succeeded to activite diapragm and improve walking streotype. Conclusion: During the cooperation with client we achieved an objective which we picked at the beginning of the therapy. The client is now interested in her disease and she want to improve her physical and psychical condition.
