Case management in the Czech republic

Abstract:

Bakalářská práce se týká tématu case management v České republice. Zaměřuje se na pojem case management, tedy na koordinovanou spolupráci specializovaných organizací s vybranými cílovými skupinami klientů. Informuje o základních pojmech souvisejících s metodou case management. Zabývá se individuálním a týmovým case managementem. V jedné kapitole také řeší problematiku cílových skupin, se kterými se můžeme v rámci této metody setkat. Dále zjišťujeme, v jakých městech České republiky je daná metoda využívána. V České republice je metoda docela zastoupena, i když není tak formalizovaná jako v zahraničí a české organizace se tak inspirují zahraničními zkušenostmi, neboť tam je case management přirozenou součástí péče. Aplikační část si klade za cíl na základě analýzy dostupných zdrojů zjistit kde, s kým a jakým způsobem je metoda case managementu využívána a zjištění zaznačit do přehledné mapy České republiky. Dalším cílem práce je zjistit se kterými cílovými skupinami odborníci v praxi pracují touto metodou.

Abstract:

The bachelor thesis relates topic case management in the Czech republic. It focuses on the concept of case management, so for coordinated cooperation between specialized organizations with selected target groups of clients. It informs about basic concepts related to the method of case management. It deals with individual and team case management. In one chapter also addresses the issue of target groups with which we in this method may encounter. Furthermore, we find in what cities in the Czech Republic, the method is used. The Czech Republic is a method of fairly represented, although not as formalized as foreign and Czech organizations and inspire foreign experience, because there is a natural part of the case management care. Application part aims based on an analysis of available resources to find out where, with whom and in what way is the method of case management and used to detect charted a clear map of the Czech Republic. Another aim is to identify target groups with which practitioners work with this method.
