Lombalgie croniche aspecifiche: confronto tra porotocollo antalgico standard di terapia fisica e protocollo personale di rinforzo muscolare isometrico

Chronic non-specific low back pain: comparison between standard antalgic protocol of physical therapy and personal protocol of isometric muscular strengthening.

Abstract:

Increasingly frequent in patology the cases of painful suffering of lumbar vertebral segment, both acute and chronic. What most impressed me in the observation of these patients is the situation that brings them together, talking about the deep muscle weakness, or the appearing of compensation antalgic posture, that day by day may change the structure of the physiological curves of the spine, both on the sagittal plane than on the frontal. Indeed we can find in literature that a insufficient physical activity and lumbar muscle weakness predisposing to reduced motion and loss of strength, endurance and coordination, showing how in patients with chronic low back pain are less developed and trofic the paraspinal muscles, containing a larger quantity of fat and having a selective atrophy of muscle fibers, because not recruited. For this reason I decided to give special attention to the comparison between the active patient muscle work (based primarily on proprioceptive training and isometric deep work) against a standard protocol of physical therapy (tecar ? therapy) directed to an antalgic objective., but according to my little experience careless to a long-term program, able to prevent painful recurrences. Often may happen that the intervertebral disc, the structure that acts as a shock absorber between two vertebral bodies, is subjected to mechanical stress together with the periarticular inert structures, that with time become damaging whether they are stress from repetitive motion whether arising from an incorrect maintained posture. Then what I believe to be necessary for good health of our spine is the right balance and alignment, always in our physiological compensation, of the various vertebral segments, obtainable reaching a strong, flexible and dynamic deep musculature, which is lumbar, thoracic or cervical; able in every activity of the subject to make "clean" motions from the biomechanical perspective, in space, of our vertebral bodies, going to stress less the inert periarticular structures (disc, ligaments, periarticular soft tissues). To do this I will create two working groups with similar common characters (age, sex, body size and clinical features), with ten patients in each protocol, which will be subjected to two different types of rehabilitation work and then observe and collect results on the progress or improvement of symptoms. After i'll collect information about the pain in the 'beginning and the end of treatment using the visual analog scale (VAS) and the Schober's test, prospecting the joint range of motion in flexion of the lumbar spine. I'll try in my experimental thesis research to verify that it is better in cases of chronic lumbar pains, use a deep muscle strengthening protocol compared to a strictly antalgic physical therapy, demonstrating how these exercises are also very useful in the improvement of a correct posture and therefore less predisposing to chronic pain. I would therefore suggest in my personal protocol a functional rehabilitation treatment of disability as a whole and not simply a temporary reduction pain of the patient, providing one kind of physical intensive and ergonomia training and providing instructions on attitudes to be taken in various everyday situations.

Sempre molto frequenti sono in patologia i casi di sofferenza (dolorosa) del segmento rachideo lombare, di varia eziologia, sia acuti che cronici. Quello che pi? mi ha colpito nell'osservazione di questi pazienti ? la situazione che li accomuna, riferendomi alla debolezza muscolare profonda, cos? come all'instaurarsi con il passare del tempo e quindi con il cronicizzarsi del dolore, di atteggiamenti posturali di compenso scorretti, che potranno o meno alterare l'assetto fisiologico delle curve rachidee ,sia sul piano sagittale che su quello frontale. Si pu? infatti ritrovare in letteratura che una scarsa attivit? fisica e una debolezza muscolare lombare siano predisponenti a una ridotta articolarit? e ad una perdita di forza, resistenza e coordinazione; evidenziando quindi come nei pazienti con low back pain(LBP, dolore lombare) cronico siano meno sviluppati i muscoli paravertebrali, contenendo una quantit? maggiore di grasso e presentando una atrofia selettiva delle fibre muscolari, perché poco reclutate. Per questo motivo ho deciso di porre particolare attenzione al confronto tra un lavoro muscolare attivo da parte del paziente (basato principalmente su un lavoro isometrico profondo e propriocettivo) e un protocollo standard di terapia fisica (tecar terapia) indirizzato ad uno scopo antalgico, ma secondo la mia piccola esperienza poco attento ad un programma a lungo termine, capace di evitare possibili recidive dolorose. Spesso pu? infatti accadere che il disco intervertebrale, quella struttura che funge da ammortizzatore tra due corpi vertebrali e che altro non fa che adattarsi e interporsi ai continui movimenti, sia sottoposto a stress di tipo meccanico insieme alle strutture inerti periarticolari, che con il passare del tempo divengono dannosi, sia che siano stress da movimento ripetuto sia che derivino da una postura mantenuta scorretta. Quello quindi che ritengo essere indispensabile per una buona salute del nostro rachide ? il giusto rapporto e allineamento, seppur sempre nei nostri compensi fisiologici, dei vari segmenti vertebrali, ottenibile raggiungendo una robusta, elastica e dinamica muscolatura profonda, lombare, toracica o cervicale che sia; capace in ogni attivit? del soggetto di far compiere movimenti "puliti", dal punto di vista biomeccanico, nello spazio, dei nostri corpi vertebrali, andando cos? a stressare meno le strutture inerti(disco, legamenti, tessuti molli periarticolari). Per fare questo creer? due gruppi di lavoro con simili caratteri comuni(fascia d'et?, sesso, corporatura e caratteristiche cliniche) , con dieci pazienti per ogni protocollo, che verranno sottoposti a due tipi di lavoro riabilitativo diverso per poi osservare e raccogliere i risultati sul progredire o meno della sintomatologia. Raccoglier? informazioni riguardanti il dolore all' inizio e alla fine del trattamento tramite la scala analogico visiva del dolore (VAS) e il test di Schober, indagante il range articolare di movimento in flessione del rachide lombare. Cercher? nella mia tesi di ricerca sperimentale di verificare come sia preferibile in casi di algie croniche lombari, utilizzare un protocollo di rinforzo muscolare profondo rispetto ad uno prettamente antalgico di terapia fisica, dimostrando come questi esercizi siano poi molto utili anche nel miglioramento di quello che ? un assetto posturale corretto e quindi meno predisponente a dolori cronici. Proporr? quindi nel mio personale protocollo di riabilitazione funzionale un trattamento della disabilit? nel suo insieme e non semplicemente una diminuzione temporanea del dolore riferito dal paziente, fornendo un tipo di training fisico ed ergonomico intensivo e fornendo istruzioni inerenti atteggiamenti da adottare nelle varie situazioni quotidiane.
