Immunoanalytical Determination of the Carcinoembryonic Antigen (CEA) on UniCel DxI 800 Analyzer, Manufacturer Beckman Coulter

Abstract:

Bakalářská práce se zabývá problematikou stále se zvyšujícího počtu nádorových onemocnění v moderní společnosti. Je věnována nádorovým markerům, jakožto laboratorním ukazatelům přítomnosti maligní transformace tkáně v lidském organismu, přičemž se soustřeďuje především na karcinoembryonální antigen (CEA) a jeho stanovení v lidském séru imunoanalytickými metodami. Cílem práce je verifikace metody stanovení CEA pomocí automatického imunoanalyzátoru UniCel DxI 800 firmy Beckman Coulter na Oddělení klinické biochemie FNO v souvislosti s ukončením produkce soupravy CEA IRMA KIT IM 2204-IM2274 výrobce Immunotech, kterou dosud laboratoř používala v rutinní diagnostice. V rámci verifikace byla ověřena opakovatelnost, mezilehlá preciznost a pravdivost a byla potvrzena použitelnost metody porovnáním výsledků těchto parametrů s hodnotami deklarovanými výrobcem. Následně byl vypočten odhad nejistoty měření. Součástí práce bylo rovněž srovnání výsledků stanovení CEA oběma metodami. Analýza byla provedena u skupiny 73 pacientů. Koncentrace CEA ve vzorcích séra byla stanovena imunoradiometrickou metodou IRMA za použití soupravy CEA IRMA KIT firmy Immunotech a na analyzátoru UniCel DxI 800. Z celkového počtu měřených vzorků byly 4 vzorky vyloučeny a zbylé výsledky stanovení byly statisticky zpracovány. Pro porovnání metod byla použita lineární regrese, Passing-Bablokova regrese a rovněž byl sestrojen Bland-Altmanův graf. Na základě statistického zpracování bylo zjištěno, že metody neposkytují shodné výsledky. Naměřené koncentrace CEA na analyzátoru UniCel DxI 800 byly přibližně o 35?60 % vyšší než koncentrace naměřené metodou IRMA Immunotech. Imunoanalyzátor UniCel DxI 800 lze použít pro stanovení CEA v rutinní praxi, avšak za podmínky změny referenčního intervalu pro hodnoty CEA.

Abstract:

The bachelor thesis deals with the issue of the continuously increasing number of cancer diseases in the modern society. It is devoted to tumor markers, as well as to the laboratory indicators that show presence of malignant transformation of tissues in a human body focusing primarily on the carcinoembryonic antigen (CEA) and its determination in human serum by means of immunoanalytical methods. The aim of the thesis is verification of the method of CEA determination with the help of the UniCel DxI 800 immunoanalyzer, manufactured by Beckman Coulter, at the Department of Clinical Biochemistry at the University Hospital of Ostrava in relation to the termination of production of CEA IRMA KIT IM 2204-IM2274 sets manufactured by Immunotech that has been used in routine laboratory diagnostics until nowadays. Repeatability, intermediate precision and verity has been verified and applicability of the method was confirmed by comparing the results of these parameters with the values declared by the manufacturer. Thereafter was calculated estimation of measurement uncertainty. A part of the study was devoted to the comparison of the results of CEA determination by both methods. The analysis was carried out in a group of 73 patients. Concentrations of CEA in serum samples were measured by IRMA immunoradiometric method using CEA IRMA KIT manufactured by the company Immunotech and by the Unicel DxI 800 analyzer. 4 samples were excluded out of the total number of measured samples and the remaining results were statistically processed. Linear regression, Passing-Bablok regression and also Bland-Altman graph has been used to compare the methods. Based on the statistical processing, it has been found out that the methods do not provide identical results. The measured concentrations of CEA on Unicel DxI 800 analyzer were approximately 35?60 % higher than those measured by IRMA Immunotech. Therefore the Unicel DxI 800 analyzer can be used for the determination of CEA in a routine practice under the condition that the reference interval for CEA values is changed.
