Community care in midwifery in the Czech Republic

Abstract:

Úvod: V období gravidity vyžaduje žena specifickou péči. Tuto péči zabezpečují porodníci a porodní asistentky v gynekologických ordinacích nebo nemocnicích, ale také komunitní porodní asistentky. Jejich práce spočívá v prenatální péči o každou těhotnou ženu a postnatální péči v období šestinedělí. Ve světě je tato péče porodní asistentky naprosto běžná. Dle platné legislativy je porodní asistentka v České republice kompetentní poskytovat péči těhotným ženám, ženám při porodu a v šestinedělí. Návštěvní služba či primární péče porodní asistentkou ale není v České republice standardně poskytovanou službou. Komunitní péče prováděná porodní asistentkou je většinou realizovaná jen v rámci těhotenského cvičení a různých kurzů pro těhotné. Pohlédneme li zpět do historie, byla komplexní péče o těhotné ženy a ženy v šestinedělí obvyklou záležitostí a povinností každé porodní asistentky. Cíl: Cílem je porovnat komunitní péči v porodní asistenci ve vybraných městech České republiky, konkrétně ve městech Plzni a Ostravě. Metodika: Výzkumný soubor je tvořen klientkami soukromých a ambulantních porodních asistentek ve městech Plzni a Ostravě (n = 191). K výzkumu byl použit standardizovaný dotazník EUROPEP, který se skládal z pěti domén, které tvořilo 23 otázek. K našemu výzkumu bylo použito 20 otázek, podle kterých klientky hodnotily svojí porodní asistentku s ohledem na to, zda jim poskytuje dostatek času, projevuje zájem nebo zda se jí dá dovolat telefonem či se objednat. Standardizovaný dotazník byl, v závislosti na stanovených cílech výzkumu, doplněn o demografická data, jako je věk, vzdělání a parita ženy. Dále o doplňující otázky týkající se služeb, které porodní asistentky nabízejí. Výzkumné šetření probíhalo od října 2011 do ledna 2012. Sběr dat probíhal v gynekologických ambulancích a centrech soukromých porodních asistentek v Ostravě a Plzni. Dotazníky byly rozdány vyškolenými porodními asistentkami instruovanými výzkumníkem a následně vloženy do uzavřených boxů. Pro srovnání spokojenosti s poskytovanou péčí byl použit neparametrický Wilcoxonův test pro dva výběry. Pro analýzu závislosti popisných znaků byl použit chi2 test a Fisherův exaktní test. Data byla zpracována statistickým programem Stata v. 10 Výsledky: Předložené výsledky této práce posuzují skupinu žen, které využívaly služby soukromé porodní asistentky v Ostravě a Plzni a svědčí pro vyšší kvalitu poskytované péče ve čtyřech doménách než u respondentek, které využívaly služby porodní asistentky pracující v gynekologické ambulanci. Ženy, které navštěvovaly soukromou porodní asistentku, nejlépe hodnotily její důkladnost a pečlivost při prenatální péči. Dále byl také prokázán vliv místa bydliště na výsledky výzkumu, kdy vyšší míru spokojenosti projevily ženy, které navštěvovaly soukromou porodní asistentku v Plzni, i přesto, že v Ostravě byly služby soukromé porodní asistentky, jako jsou předporodní kurzy nebo návštěvní služba více využívané než v Plzni. Také vzdělání se ukázalo jako důležitý ukazatel při využívání služeb soukromé porodní asistentky, kdy ženy využívající služeb soukromé porodní asistentky korespondovaly vyšším nebo vysokoškolským vzděláním oproti ženám, které navštěvovaly porodní asistentku pracující v gynekologické ambulanci. Závěr: V této práci byly identifikovány silné a slabé stránky péče porodní asistentky tak, jak je vnímaly těhotné ženy jako jejich klientky. Studie by mohla přispět ke zvýšení spokojenosti klientek, které služby porodních asistentek využívají.

Abstract:

Introduction: A woman requires specific care during pregnancy. This care is provided with obstetricians and midwives in gynaecological surgeries or hospitals, but also with community midwives. Their work lies in prenatal care for every pregnant woman and postnatal care in puerperium. This community midwife?s care is completely common worldwide. According to the valid legislation of the Czech Republic this midwife is competent to provide care to pregnant women, women during delivery or in puerperium. However, visiting service or primary care of this midwife is not still a standard service in the Czech Republic. The community care carried out by the midwife is mostly done only within pregnancy exercise and various kinds of courses for pregnant. If we look back to history, we learn that complex care for pregnant women and women in puerperium has always been matter and duty of every ordinary midwife. Aim: The aim of this thesis is to compare community care in pregnancy assistance in chosen cities of the Czech Republic, namely in Plzeň and Ostrava. Methodology: The research sample is made by clients of private and outpatient midwives in Plzeň and Ostrava (n = 191). A standardised questionnaire EUROPEP has been used for this research comprising five domains formed by twenty-three questions. Twenty questions of them were used for our survey. The clients there evaluated their midwife with regard to the sufficient amount of offered time, shown interest in them, or if she is available on the phone. The standardised questionnaire has been supplemented by demographic data as age, education or parity of woman, depending on the fixed targets of the research. There were also some additional questions concerning the services offered by midwives. The research investigation was going on from October 2011 to January 2012. The data collection took place in gynaelocogical ambulancies and centres of private midwives in Ostrava and Plzeň. The questionnaires were distributed by trained midwives, instructed by the researcher and they were subsequently inserted to closed boxes. The non-parametric Wilcoxon test for two choices has been used for the care satisfaction?s comparison. The Chi2 test and the Fisher?s exact test have been used for analysis of the dependency on descriptive characters. The data have been processed by statisctical programme Stata v. 10. Results: The presented results of this work assess group of women who used the services of private midwife in Ostrava and Plzeň and give evidence for higher quality of provided care in four domains than of respondents using midwife working in gynaelogical ambulance. The women who visited private midwife evaluated her thoroughness and carefulness during prenatal care the best. Then, the influence of the place of residence has been proved influential for the results, while higher rate of satisfaction was demonstrated by women who visited private midwife in Plzeň, although in Ostrava the services of private midwives (such as antenatal courses or visiting service) were used more than in Plzeň. Education also showed as an important indicator during using services of private midwife, when these women corresponded with higher or university education comparing to women visiting her midwife in gynaecological ambulance. Conclusion: The strengths and weaknesses of midwife?s care have been identified in this thesis in the way how the pregnant women perceived them as their clients. The thesis can contribute to an increase of the satisfaction of the women using the midwife?s service.
