Effect of Hyperbaric Oxygen Therapy in the treatment of anaerobic infections

Abstract:

Úvod Anaerobní bakterie do organismu vstupují z vnějšího prostředí kontaminací rány např. půdou, nebo vnitřní invazí z přirozené bakteriální flóry. Clostridií bylo dosud izolováno více než 150 druhů, přičemž pouze několik z nich působí patogeneticky u člověka. Pro šíření anaerobní infekce je důležitá individuální citlivost hostitele. Mezi anaerobní infekce patří: nekrotizující infekce, klostridiové infekce, gangréna. Hyperbarická kyslíková oxygenoterapie (dále HBO) je krátkodobá inhalace vysoké dávky kyslíku,který je dodáván systémově přes dýchací cesty a krev za vyššího tlaku, než je tlak atmosférický. Metodika Data získávala autorka z dokumentace pacientů léčených v Centru hyperbarické medicíny v Městské nemocnici Ostrava od ledna 2007 do ledna 2011. Autorka zjišťovala aktuální stav defektu u pacientů před začátkem léčby hyperbarickou oxygenoterapií a po páté aplikaci HBO. Sledovala změny ve velikosti defektu, přítomnosti zápachu a množství sekrece, kterou hodnotila počty převazů defektu za 24 hodin. Do studie byli zařazeni pacienti s diagnózou: nekrotizující fasciitída, klostridiová infekce, flegmóna, u kterých byly tyto diagnózy určujícím parametrem pro léčbu HBO, byli také ošetřeni chirurgicky a léčeni antibiotiky. Pro statistické vyhodnocení byla pro popis dat použita popisná statistika. Pro srovnání sledovaných parametrů před aplikací HBO a po pěti aplikacích HBO byl použit párový t-test a test symetrie a pro srovnání sledovaných parametrů ve vztahu k jednotlivým diagnózám byla použita analýza rozptylu (ANOVA) a ?2 test. Výsledky Při porovnání všech sledovaných parametrů tj. velikosti defektu, přítomnost zápachu a intenzity sekrece, bylo zjištěno, že je u všech statisticky významný rozdíl před a po 5. aplikaci HBO. Závěr Hyperbarická oxygenoterapie se stala součástí léčby anaerobních infekcí, ale tvrzení, že základním kamenem léčby anaerobních infekcí je právě tato metoda, by bylo mylné. Důkazní úroveň studií EBM (evidence based medicine) zabývajících se touto problematikou je klasifikována jako úroveň 3 tj. že existují jen slabé důkazy a tato léčba je doporučována na základě odborného konsensu nebo nekontrolované studie a dále, že by ?měla? nebo ?může? být aplikována u vhodných pacientů. Podmínkou pro více než půl století aplikovanou léčbu hyperbarickou oxygenoterapií u těchto infekcí, je agresivní, následně tkáň šetřící débridement a širokospektrální antibiotická terapie. Potenciální terapeutické přínosy hyperbarické oxygenoterapie se vztahují k šíření kyslíku prostřednictvím krve do okolí zraněných nebo infikovaných hypoxických tkání a tím napomáhá k přežití životaschopné tkáni a vytvoří jasnou hranici mezi zdravou a nekrotickou tkání, má rovněž sekundární farmakologické účinky na zánět a hojení ran. Hlavním cílem této práce bylo zjistit, zda u pacientů, kteří tuto léčbu podstoupili došlo ke zlepšení lokálního nálezu defektu. Cíl této práce byl splněn, autorka zjistila, že je zlepšení v lokálním nálezu defektu po 5. aplikaci HBO.

Abstract:

Introduction Anaerobic bacteria enter the body from the external environment such as contamination of wounds for example by soil or internal invasion of the natural bacterial flora. Until now there has been isolated more than 150 types of Clostridia,but only few of them are pathogenetic in humans. Individual host susceptibility is important for the spread of anaerobic infections. Anaerobic infections are necrotizing infections, clostridial infection and gangrene. HBO is a short-term inhalation of high doses oxygen which is delivered systemically through the respiratory tract and the blood at a higher pressure than the atmospheric pressure. Methodology The author obtained data from records of patients treated at the Hyperbaric Medical Center at the Municipal Hospital in Ostrava within the years 2007 - 2011. The author examined the current status of the defect in patients before hyperbaric oxygen therapy treatment and oxygen therapy after the fifth application of the therapy. The author monitored changes in the size of the defect, the presence of odors and the amount of secretion, which evaluated accorrding to the number of dressing changes for the period of 24 hours. The study included patients with a diagnosis: necrotizing fasciitis, clostridial infection, cellulitis, where these diagnoses were taken as the main parameter for the treatment of hyperbaric oxygen therapy, surgical treatment or treatment with antibiotics. Descriptive statistics were used for statistical evaluation. For comparison of the observed parameters prior to hyperbaric oxygen therapy and after the five applications of the hyperbaric oxygen therapy there were used paired t-test and test for symmetry and to compare monitored parameters in relation to individual diagnoses there was used analysis of variance (ANOVA) and test. Resuts A comparison of the observed parameters, ie the size defects, the presence and intensity of odor secretion, it was found statistically significant difference before and after the fifth application of hyperbaric oxygen therapy. ? Conclusion Hyperbaric oxygen therapy subsequently became part of the treatment of anaerobic infections, but the argument that the cornerstone of treatment of anaerobic infections is precisely this method, it would be wrong. Probative value of evidence based medicine dealing with this issue is classified as level 3, ie that there is only weak evidence, and this treatment is recommended based on expert consensus or uncontrolled studies and further that it should or may be applied in suitable patients . Prerequisite for more than half a century of the treatment with hyperbaric oxygen therapy for these infections is aggressive, then tissuesaving debridement and broad spectrum antibiotic therapy. Potential therapeutic benefits of hyperbaric oxygen therapy are related to the diffusion of oxygen through the blood to the area of injured or infected hypoxic tissues, and thus contributes to the survival of viable tissue and creates clear line between healthy and necrotic tissue and it has also secondary pharmacological effects on inflammation and wound healing. The main objective of this work was to determine whether patients who have undergone this treatment has improved defect. The aim of this work was met, the author found that the improvement of local defects before and after fifth hyperparc oxygen therapy.
