Water birth versus conventional vaginal birth

Abstract:

Cíl: Cílem diplomové práce bylo zjistit rozdíly v průběhu porodu mezi porodem do vody a konvenčním vaginálním porodem u žen s nízkorizikovým těhotenstvím. Výzkum se zaměřil na jednotlivé fáze porodu, četnost využití analgezie, výskyt porodního poranění, poporodního krvácení, novorozeneckou adaptaci, lékařské zásahy a poporodní komplikace. Metodika: Výzkum probíhal formou retrospektivní analýzy 200 zdravotnických dokumentací - 100 žen po konvenčním porodu a 100 žen po porodu do vody. Kritéria zahrnovala těhotenství s nízkým rizikem, jednočetnou graviditu v termínu a aktivní fázi porodu. Data byla statisticky zpracována pomocí deskriptivní statistiky, Chi-kvadrát testu, Mann-Whitneyho testu a dalších testů při hladině významnosti ? = 0,05. Výsledky: Výsledky ukázaly, že porod do vody byl spojen s nižší četností aplikace analgezie (zejména s absencí EDA), nižším výskytem porodních poranění (např. epiziotomií), kratší druhou dobou porodní a nižší potřebou lékařských zásahů. Naopak v délce první a třetí doby porodní, množství poporodní krevní ztráty, výskytu poporodních komplikací a v Apgar skóre novorozenců nebyly zjištěny statisticky významné rozdíly mezi oběma skupinami. Závěry: Porod do vody se na základě výsledků studie jeví jako bezpečná a přínosná alternativa ke konvenčnímu vaginálnímu porodu u žen s nízkorizikovým těhotenstvím. Byl spojen s nižší potřebou analgezie, kratší druhou dobou porodní, menším výskytem porodních poranění a nižší mírou lékařských zásahů. Zároveň nebyly zaznamenány negativní dopady na zdravotní stav matky ani novorozence. Tyto poznatky podporují možnost širšího využití porodů do vody jako šetrnější formy vedení porodu, která respektuje přirozené procesy a preference rodiček.

Abstract:

Aim: The aim of this thesis was to identify differences in the course of labour between water births and conventional vaginal deliveries in women with low-risk pregnancies. The research focused on individual stages of labour, the frequency of analgesia use, the incidence of birth injuries, postpartum bleeding, neonatal adaptation, medical interventions, and postpartum complications. Methodology: The research was conducted through a retrospective analysis of 200 medical records - 100 women after conventional delivery and 100 women after water birth. Inclusion criteria encompassed low-risk pregnancy, singleton term gestation, and the active phase of labour. The data were statistically evaluated using descriptive statistics, the Chi-square test, the Mann-Whitney U test, and other appropriate tests with a significance level set at ? = 0.05. Results: The results showed that water birth was associated with a lower frequency of analgesia use (particularly with no use of epidural analgesia), fewer birth injuries (e.g., episiotomies), a shorter second stage of labour, and reduced need for medical interventions. On the other hand, no statistically significant differences were found between the two groups in the length of the first and third stages of labour, volume of postpartum blood loss, incidence of postpartum complications, or neonatal Apgar scores. Conclusions: Based on the findings of this study, water birth appears to be a safe and beneficial alternative to conventional vaginal delivery in women with low-risk pregnancies. It was associated with less need for analgesia, a shorter second stage of labour, fewer birth injuries, and a lower rate of medical interventions. No adverse effects were observed on the health of the mother or the newborn. These findings support the potential for broader implementation of water births as a more natural and respectful form of childbirth, aligning with the preferences of birthing women.
