The influence of the form of delivery and obstetric tears on a woman's sexuality

Abstract:

Cíl: Cílem práce bylo zjistit, zda má způsob porodu a porodní poranění vliv na ženskou sexualitu. Metodika: Základní výzkumný soubor tvořilo 99 žen ve věku 18 ? 40 let, které byly po porodu v časovém rozmezí 6 měsíců až 3 roky. V souboru bylo zastoupeno 70 žen po spontánním vaginálním porodu, 17 žen po císařském řezu a 12 žen po operativním vaginálním porodu. Kontrolní soubor obsahoval 80 žen stejné věkové skupiny, které zatím nerodily. K měření sexuality byl použit standardizovaný dotazník Female sexual function index (FSFI). Výsledky: Ženy po porodu mají sníženou sexuální touhu, ale dosahují lepších výsledků v dimenzi orgasmu než ženy, které nerodily. Při porovnávání jednotlivých způsobu porodů, bylo zjištěno, že nejnižší míru bolesti a nejméně problémů s dosažením a udržením lubrikace mají ženy po porodu císařským řezem. U žen po operativních vaginálních porodech byl zjištěn nejnižší výskyt orgasmů a nejmenší spokojenost se schopností orgasmu dosáhnout. V porovnání s ženami po spontánních vaginálních porodech, byl u těchto žen vyšší výskyt bolesti při penetraci a během styku a rovněž míra této bolesti byla významně vyšší. Při srovnání s ženami po císařském řezu, ženy po operativních vaginálních porodech jsou méně spokojeny se svou sexuální vzrušivostí, se sexuálním vztahem s partnerem i s mírou jejich emoční důvěrnosti. U žen po epiziotomií byl prokázán častější výskyt bolesti během pohlavního styku a signifikantně vyšší míra této bolesti než u žen s rupturou hráze I. stupně nebo žen bez poranění hráze. Závěr: Výsledky poukazují na skutečnost, že způsob porodu a porodní poranění mají vliv na sexuální život ženy.

Abstract:

Aim: The aim of this thesis was to find out whether the mode of delivery and the birth injury can influence the female sexuality. Methods: The basic research sample consisted of 99 women in the age of 18-40 years who delivered 6 months to 3 years ago. The sample contained 70 women after the spontaneous vaginal delivery, 17 women after the section caesarean and 12 women after the operative vaginal delivery. The control sample included 80 women of the same age who have not given birth yet. The standardized Female sexual function index (FSFI) questionnare was used for the purpose of measuring of sexuality. Results: The women after delivery have lowered sexual desire but they achieve better results in the area of orgasms than the women who have never given birth. It was found out by comparison of the particular types of deliveries that the women after section caesarean have the lowest degree of pain and the lowest number of problems with reaching and keeping lubrication. It was discovered that the women after operative vaginal delivery have the lowest occurence of orgasms and the lowest level of satisfaction with the ability to reach orgasm. When compared with the women after vaginal delivery the higher occurence of pain during penetration and intercourse was discovered and also the degree of pain was significantly higher. Compared with the women after caesarean section, the women after operative vaginal delivery are less satisfied with their sexual arousal, with sexual relationship with their partners as well as with the level of the emotional intimacy. In case of the women after episiotomy the more frequent occurence of pain during the intercourse and its significantly higher degree were proved than in case of the women with perineal rupture of I. degree or women without perineal injury. Conclusion: The results indicate that the mode of delivery and birth injuries can influence the female sexual life.
