Mutation in the NOD2/CARD15 gene - prediction of the surgical treatment in Crohn’s disease?

Abstract:

Úvod. Crohnova nemoc (CN) patří mezi chronické onemocnění s doposud neobjasněnou etiologií. Cílem práce bylo zjistit využití genetického vyšetření (NOD2/CARD15) u pacientů s CN na predikci chirurgické léčby (najít agresivní typ nemoci profitující z časné chirurgické léčby). Druhým záměrem bylo stanovit, jakým způsobem chirurgická léčba ovlivňuje pooperační kvalitu života (quality of life - QoL) u pacientů s touto chorobou. V experimentální části jsme se snažili určit přínos peroperačního využití konfokální laserové mikroskopie (KLM) a to zvláště na zjištění pozitivity resekční linie aktivním zánětem CN (pro možný vliv na brzkou recidivu CN). Podobné využití perioperační KLM zatím ve světě publikováno nebylo. Metody. Do studie s genetickým vyšetřením byli zařazeni pacienti, kteří na CHK FN Brno podstoupili operaci z důvodu CN v letech 2010-2016. Kontrolní skupinu (pacienti s méně agresivním fenotypem nemoci) představovali pacienti s CN, kteří tuto chorobu mají diagnostikovanou minimálně 5 let nebo déle a během života nevyžadovali nutnost chirurgické intervence. Další kontrolní skupinu představovali zcela zdraví jedinci. V druhé klinické části, kde jsme porovnávali QoL, byli zahrnuti pacienti, kteří podstoupili na naší klinice střevní resekci z indikace CN v letech 2010-2016. Pacienti vyplnili standardizovaný dotazník k měření QoL v předoperačním období a po operaci s odstupem 2 měsíců. Kontrolní skupiny představovali pacienti s CN, kteří nepodstoupili resekci střeva a zcela zdravá kohorta osob. Do experimentální části byli zařazeni pacienti s CN, kteří byli indikováni k ileocekální resekci v letech 2016-2017. U těchto pacientů byla provedena peroperační KLM v místě infiltrátu terminálního ilea a céka, resekční linie tlustého a tenkého střeva. Obrazy byly porovnávány s výsledným histologickým vyšetřením. Výsledky. Do genetické části práce bylo celkem zařazeno 117 operovaných pacientů s CN, 77 pacientů s CN bez operace a kontrolní zdravou kohortu tvořilo celkem 30 osob. Pacienti s alespoň jednou mutací v genu mají 1,96krát větší riziko operačního řešení pro CN než pacienti bez mutace. Pacienti s 2 a více mutacemi měli častější lokalizaci nemoci L3, tito pacienti byli obecně operovaní v mladším věku, dříve od stanovení diagnózy CN a zároveň u každého pacienta ve sledovaném souboru se 2 a více mutacemi byla provedena resekce části ilea. V části hodnocení QoL bylo zahrnuto celkem 132 operantů pro CN, 83 pacientů s CN bez operace a 104 zdravých jedinců. Pooperační zlepšení QoL bylo zjištěno celkem u 104 operovaných pacientů (78,8%), u 2 pacientů nebyla zaznamenána žádná změna v QoL (1,5%) a u 26 pacientů (19,7%) bylo zjištěno zhoršení pooperační QoL. KLM byla provedena celkem u 12 pacientů. U všech pacientů bylo realizováno vyšetření ze serózy střeva, kdy byly stanoveny jednotlivé charakteristické obrazy pro zánět a klidovou tkáň. Zároveň jsme u 6 pacientů provedli vyšetření i ze sliznice. Závěr. V operované skupině pacientů s CN bylo významně vyšší zastoupení alespoň jedné mutace v genu oproti neoperované skupině. Pro pacienty s 2 a více mutacemi byl charakteristický agresivnější průběh nemoci. Tato skupina pacientů by mohla profitovat z top-down či časné chirurgické terapie. Další důležité zjištění bylo, že pozitivní resekční linie aktivním zánětem CN zvyšuje až 3,83krát riziko časné pooperační rekurence nemoci v anastomóze (avšak nezávisle na genetické mutaci). Prokázali jsme signifikantní zlepšení celkové QoL po střevní resekci u pacientů s CN. Pohlaví bylo vyhodnoceno jako jediný statisticky významný faktor ovlivňující pooperační QoL. Abstract:

Background. Crohn’s disease (CD) belongs to chronic diseases whose etiology remains uncertain. The aim of this work was to identify how genetic testing (NOD2/CARD15) can be used in patients with CD to predict the need for surgical treatment (to define an aggressive type of disease that can profit from early surgery). Another aim was to assess how surgical treatment influences the patients’ postoperative quality of life (QoL). In the experimental part, we tried to determine the benefits of using confocal laser microscopy (CLM) intraoperatively, especially in detection of resection margin positivity of active CD (because of a possible influence on early CD recurrence). A similar use of CLM has not been internationally published yet. Methods. The patients who were subjected to genetic testing were patients who had undergone a surgery due to CD at the Department of Surgery University hospital in Brno Bohunice between 2010-2016. The control group consisted of patients with CD who had been diagnosed with CD at least 5 years prior to the testing and had not required any surgical intervention. The second control group were healthy subjects. In the clinical part which compared the quality of life before and after surgery, we studied patients who had undergone a bowel resection surgery at our department due to CD between 2010-2016. The patients filled in a standardized questionnaire to assess QoL in the preoperative period and the postoperative period after a 2-month interval. The control groups were patients with CD who had not undergone bowel resection and a healthy cohort. The experimental part included patients with CD who had been indicated for ileocecal resection in the years 2016-2017. These patients were subjected intraoperatively to CLM in the area of the terminal ileum and cecum, resection margin of the small and large intestine. The images were compared with the final histological examination. Results. In the genetic part, there were 117 operated patients with CD, 77 patients with CD without an operation and 30 healthy subjects. For patients with at least one genetic mutation, the risk of the necessity of surgical treatment of CD is 1.96 times higher than for patients with no mutation. Patients with two or more mutations had a more frequent localization of the L3 type of the disease. These patients were generally operated on at a younger age, a shorter time after being diagnosed with CD and each patient in the group with two or more mutations had a partial resection of the ileum. In the QoL evaluation part, there were 132 operated patients with CD, 83 patients with CD without an operation and 104 healthy subjects. 104 of the operated patients had experienced a postoperative improvement of the QoL (78.8%), 2 patients had not registered any changes in QoL (1.5%) and 26 patients (19.7%) had experienced a worsening of their postoperative QoL. CLM was performed on 12 patients. All patients underwent an examination of the bowel serosa and characteristic images of inflamed and quiescent tissue were identified. Furthermore, 6 of the patients also had mucosa examination. Conclusions. The group of operated patients with CD had a significantly higher distribution of at least one genetic mutation as opposed to the non-operated group. In patients with two or more mutations, the development of the disease was more aggressive. This group of patients might profit from the top-down or early surgical therapy. Another important finding was that the positive resection margin of active CD increases the risk of early postoperative recurrence in anastomosis up to 3.83 times (however, independent of a genetic mutation) We registered a significant improvement of the overall QoL after bowel resection in patients with CD. h the histological findings, which means that an intraoperative identification of an active inflammation is possible. Identifying a negative resection margin by CLM could have a major impact on the reduction of CD recurrence in anastomosis.
