Effective factors of ritual ayahuasca use in the treatment of addictions in the psychotherapeutic context

Abstract:

Ayahuasca je tradiční jihoamerický nápoj s psychedelickými účinky. Jako součást tradiční amazonské medicíny je v rituálním kontexu používán v kombinaci s psychoterapií k rehabilitaci závislých v centru Takiwasi v Peru. Zkušenosti z Takiwasi i jiné studie dlouhodobých uživatelů naznačují terapeutický potenciál ayahuascy pro léčbu závislostí. Cílem této práce bylo prozkoumat, jakým způsobem ayahuasca terapeuticky působí při léčbě závislostí, s důrazem na psychologické mechanismy. Dílčím cílem bylo zabývat se rolí psychoterapie v tomto kontextu. Studie měla kvalitativní design, hlavním zdrojem dat byly rozhovory s 9 pacienty centra Takiwasi, doplňková data tvořilo více než 150 protokolů z ayahuascových a psychoterapeutických sezení. Na základě rozhovorů byly identifikovány terapeuticky účinné faktory ayahuascy. Některé z nich jsou známé z výzkumů psychoterapie: psychická očista, přístup do nevědomí, vhledy, tvarování chování, naděje, akceptace a uvědomování. Jiné jsou specifické pro práci s ayahuascou nebo obecně změněnými stavy vědomí: fyzická a spirituální očista, zprostředkování léčby, kontakt s jinými úrovněmi existence, mimořádná zkušenost, učení pomocí prožitku a vizí, autentické prožitky radosti a tvarování na psychoterapii, Terapeut ve vztahu k ayahuascovým sezením vystupoval jako facilitátor práce s ayahuascou a průvodce. Tato studie podpořila názor, že by ayahuasca mohla být terapeuticky účinná při léčbě závislostí. Ukázala na možné především psychologické mechanismy jejího účinku prostřednictvím zmapování jejích terapeuticky účinných faktorů z pohledu pacientů Takiwasi. Ayahuasca se ukázala být možným facilitátorem psychoterapie, zároveň ale i psychoterapie facillitátorem sezení s ayahuascou. Obě metody se zdají být nejúčinnější při vzájemné spolupráci.

Abstract:

Ayahuasca is a traditional South American psychedelic brew. Takiwasi is a center for the rehabilitation of drug addicts in Peru, which uses a combination of psychotherapy and traditional Amazonian medicine, including ritual use of ayahuasca. Experience from Takiwasi and other studies of long-term users suggest a therapeutic potential of ayahuasca in the treatment of addiction. The main aim of this study was to explore the mechanism of therapeutic action of ayahuasca in the treatment of addictions. The secondary aim was to consider the role of psychotherapy in this context. The study employed a qualitative design: the main source of data were interviews with 9 patients from Takiwasi and additional data were collected from more than 150 reports of ayahuasca and psychotherapy sessions. Based on the interviews, therapeutically effective factors of ayahuasca treatment were identified. Some of them are known from psychotherapy research: psychic cleansing, access to the unconscious, insights, shaping of behavior, hope, acceptance and awareness. Others are specific to work with ayahuasca, or altered states of consciousness in general: physical and spiritual cleansing, mediation of therapy, contact with other levels of existence, extraordinary experience, learning through direct experience and visions, authentic experience of joy and shaping to therapy. In this specific context, the therapist emerges as a facilitator of work with ayahuasca and guide. This study supported the notion that ayahuasca can be therapeutically effective in the treatment of addictions. Ayahuasca was shown to be a potential facilitator of psychotherapy, psychotherapy appeared as a facilitator of ayahuasca sessions as well.
