Trends in Dietology focusing on Therapeutic Diet

Abstract:

Bakalářská práce se zabývá klasickou diabetickou dietou a její nově akceptovanou variantou s redukovaným obsahem sacharidů. Více než 80 % diabetiků 1. typu nedosahuje cílů, co se týče glykémie a glykovaného hemoglobinu. Narůstá počet nemocných s nedostatečnou metabolickou kontrolou cukrovky a nemocných s obezitou, a to i přes neustálé zdokonalování léčby cukrovky a používání nových technologií. Cílem této práce je porovnat vliv nízkosacharidové diety (LCD) s obsahem 100 g sacharidů na den a vysokosacharidové diety (HCD) s obsahem 200 g sacharidů na den na glykémii v krátkém čase. Nízkosacharidové diety jsou legitimní a potenciálně efektivní možností léčby nemocných s diabetem nebo s obezitou. První, teoretická část popisuje cukrovku jako nemoc a uvádí teorii, která s ní souvisí, dále obsahuje informace o diabetické dietě, o jednotlivých živinách v ní obsažených a definuje nízkosacharidovou dietu. Ve druhé, analytické částí je porovnání hodnot glykémií za období 28 dní u specifického souboru čtyř různých nemocných s cukrovkou. U třech nemocných jsou prezentovány výsledky glykémií na dvou variantách diet, s obsahem 200 g a 100 g sacharidů na den. A u jedné těhotné diabetičky na jedné variantě diety 100 g sacharidů na den. Ve třetí, návrhové části je návrh a popis preventivních programů, hrazených ze zdravotního pojištění, které mají pomoct nemocným s cukrovkou, co se týče edukace a diety. Shrnutím je, že snížení sacharidů v dietě vedlo k nižšímu výskytu hypoglykémii a snížení variability glykémie u nemocné s diabetem 1. typu, jedná se o moji osobní zkušenost. U nemocného s diabetem 2. typu bylo výsledkem snížení průměrné glykémie a u nemocné s prediabetem zlepšení hodnot glykémií hlavně po jídle. Těhotná diabetička byla celou dobu na LCD dietě výborně kompenzovaná a nevyskytly se žádné komplikace. V případě LCD je výzkum, který zatím existuje omezený, ale informace, které jsou v současnosti k dispozici, prokazují pozitivní účinek této diety na zlepšení kontroly glykémie. Tento závěr je v souladu se závěrem bakalářské práce.

Abstract:

The bachelor thesis deals with the diabetic diet and its newly accepted variant with a reduction of carbohydrate intake. More than 80 % of individuals with Type 1 Diabetes do not achieve their glycaemic target. Despite the improvements in therapy and technology, the current trend is an increase in obesity rates and impaired metabolic control in people with diabetes. The aim of this thesis is to compare the effects of a low-carbohydrate diet (LCD 100 g carbohydrate per day) and a high-carbohydrate diet (HCD 200 g carbohydrate per day) on glycaemic control in a short-term. Low-carbohydrate diets are increasingly being used to help patients with obesity as well as in people with Type 2 diabetes. The LCD is a legitimate and potentially effective treatment option for those with diabetes or obesity. The first part is theoretical and describes the theory of diabetes as a disease and its characteristics and also includes theoretical information about the diabetic diet, information about nutrients, and description of a low-carbohydrate diet. The analytical part contains a comparison of glycaemia in four different patients with diabetes. In three patients, two variants of diets were tested- LCD with 100 g of carbohydrate per day and 200 g of carbohydrate per day - for a period of 28 days. One pregnant woman with diabetes was tested only with the LCD with 100 g of carbohydrates per day. The recommendation part includes the options to help patients with diabetes in terms of education and to help them with a dieting in preventative programs. The programs would be free of charge with the possibility to be covered by healthcare insurance, if the patients achieve target results during the treatment. In conclusion, the restriction of carbohydrate intake reduced time spent in hypoglycaemia, glycaemic variability in Type 1 diabetes and decreased average glycaemia in Type 2 diabetes. The pregnant diabetic was perfectly compensated on the LCD diet throughout the experiment and there were no complications. In the case of the LCD, exists only a limited quantity of research, but the information available shows a positive effect and shows a potential to improve glycaemic control and metabolic health in people with diabetes. This conclusion agrees with the conclusion of my bachelor thesis.
