Abstract:

Úvod: Přetížení svalů rotátorové manžety ramenního kloubu v kombinaci s omezenou mobilitou hrudní páteře je častým obrazem u overhead sportů, jako je vzpírání. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u vzpěračského sportu s využitím fyzioterapeutických metod, postupů a konceptů u pacienta s bolestí ramene a zad. Metodika: V hlavní části práce je zpracována kazuistika 51letého vzpěrače, který trpí již několik let bolestí pravého ramene a zad. Doposud nebyl vyšetřen žádným lékařem ani fyzioterapeutem. Na základě vstupního kineziologického rozboru byl navržen krátkodobý fyzioterapeutický plán, který byl realizován v podobě 8 terapií, v rozmezí 8 týdnů. Pro terapii byly využity koncepty DNS, PNF, ACT, Spiraldynamik? a terapeutický koncept Jarmily Čápové. Po 4. terapii bylo provedeno kontrolní vyšetření vybraných parametrů pro kontrolu zdravotního stavu. Po osmé terapii byl proveden výstupní kineziologický rozbor, na jehož základě byl poté sestaven dlouhodobý fyzioterapeutický plán. Výsledky: Fyzioterapeutickou intervencí se podařilo zvětšit rozsah pohybu v pravém ramenním kloubu do FLX (flexe), ABD (abdukce), VR (vnitřní rotace) a v levém ramenním kloubu do ABD a ZR (zevní rotace). V levém kyčelním kloubu byl zvětšen rozsah pohybu do ABD. Byla odstraněna kloubní blokáda levého horního SI (sacroiliaca) kloubu v předozadním směru, 3. a 4. žebra vpravo, střední hrudní páteře do EXT (extenze) a dolní hrudní páteře do rotace. Byla zvětšena svalová síla do FLX pravého ramenního kloubu, dále do ABD a ZR kyčelních kloubů, ABD lopatky s rotací a FLX trupu s rotací. Došlo k normalizaci tonu m. subscapularis vpravo, m. teres minor vpravo, m. infraspinatus vpravo a šikmých břišních svalů. Jizvy na zádech byly ošetřeny měkkými technikami a jsou posunlivé vůči ostatním tkáním. Hybné stereotypy ABD v ramenním kloubu, stereotyp kliku, EXT v kyčelním kloubu byly upraveny a probíhají ve správném svalovém zapojení. Došlo k ošetření zkráceného svalu m. piriformis a m. levator scapulae. Závěry: Volbou vhodných fyzioterapeutických metod a postupů došlo ke zlepšení pacientova zdravotního stavu a umožnění plného tréninkového nasazení.

Abstract:

Introduction: Overloading the rotator cuff muscles of the shoulder, combined with limited mobility of the thoracic spine, is a common picture in overhead sports such as weightlifting. The objective: The aim of the bachelor thesis is to contribute to the solution of physiotherapy in weightlifting sports using physiotherapeutic methods, procedures and concepts in patients with shoulder and back pain. Methodology: The main part of the work deals with a case report of a 51-year-old weightlifter who has been suffering from right shoulder and back pain for several years. So far, he has not been examined by any doctor or physiotherapist. Based on the initial kinesiological analysis, a short-term physiotherapy plan was proposed, which was implemented in the form of 8 therapies, lasting 8 weeks. The concepts DNS, PNF, ACT, Spiraldynamik? and the therapeutic concept of Jarmila Čápová were used for therapy. After the 4th therapy, a control examination of selected parameters for health control was performed. After the eighth therapy, a final kinesiological analysis was performed, on the basis of which a long-term physiotherapy plan was then compiled. Results: Physiotherapeutic intervention succeeded in increasing the range of motion in the right shoulder joint to FLX (flexion), ABD (abduction), VR (internal rotation) and in the left shoulder joint to ABD and ZR (external rotation). In the left hip joint, the range of motion to the ABD was increased. The joint blockage of the left upper SI (sacroiliaca) joint in the anteroposterior direction, the 3rd and 4th ribs on the right, the middle thoracic spine into EXT (extension) and the lower thoracic spine into rotation were removed. Muscle strength was increased in the FLX of the right shoulder joint, then in the ABD and ZR of the hip joints, the ABD scapula with rotation and the FLX torso with rotation. The tone of the subscapularis muscle on the right, the teres minor muscle on the right, the infraspinatus muscle on the right and the oblique abdominal muscles were normalized. The scars on the back have been treated with soft techniques and are shifting towards other tissues. The driving stereotypes of ABD in the shoulder joint, the stereotype of the crank, and the EXT in the hip joint have been modified and are carried out in the correct muscle connection. The shortened muscle of the piriformis and levator scapulae was treated. Conclusions: The choice of appropriate physiotherapeutic methods and procedures has improved the patient's health and enabled full training.
