The effect of respiratory physiotherapy on weaning from mechanical ventilation

Abstract:

Záměrem této diplomové práce bylo porovnat, jaký vliv mají různé metody respirační fyzioterapie na úspěšnost a délku procesu odpojování od umělé plicní ventilace - weaningu. Konkrétně se jednalo o trénink nádechových svalů pomocí trenažéru Threshold, podporu hygieny dýchacích cest a expektorace pomocí trenažéru Acapella a běžně prováděnou manuální respirační fyzioterapii. Dílčím cílem práce bylo zhodnotit, jakým způsobem tyto metody respirační fyzioterapie ovlivní hodnoty maximálních okluzních ústních tlaků, které svědčí o svalové síle dýchacích svalů. Výzkum probíhal na Klinice anesteziologie, resuscitace a intenzivní medicíny Fakultní nemocnice Ostrava. Kritériem pro zařazení bylo připojení na umělou plicní ventilaci minimálně 48 hodin, zahájený proces weaningu, indikace lékařem k dechové rehabilitaci a spolupracující pacient. Celkem bylo do výzkumu zařazeno 14 pacientů, dokončilo ho však pouze 9 z nich. Jednalo se o 5 žen a 4 muže, průměrný věk 70 ? 6,5 roků. Průměrná délka umělé plicní ventilace před zahájením weaningu byla u pacientů 19,8 dní a průměrná délka prováděné respirační fyzioterapie pak 10,6 dní. Pro měření hodnot okluzních ústních tlaků přes tracheostomickou kanylu byl použit přístroj Micro RPM (MD Spiro - Micro Direct, Lewiston, USA). Statistická analýza získaných dat byla provedena pomocí Kruskal-Wallisova testu a T-testu. Z výsledků práce pak vyplývá, že nebyl zjištěn statisticky významný rozdíl ve vlivu zkoumaných metod respirační fyzioterapie na úspěšnost ani na délku weaningu. Nicméně respirační fyzioterapie zaměřená na trénink nádechových svalů pomocí trenažéru Threshold vedla ke statisticky významnému zvýšení hodnot nádechových okluzních ústních tlaků, a tedy ke zvýšení svalové síly těchto svalů. V případě manuální respirační fyzioterapie pak došlo ke statisticky významnému zvýšení hodnot výdechových ústních tlaků.

Abstract:

The objective of this master's thesis was to compare the effect of different methods of respiratory physiotherapy on the length and outcome of the weaning from mechanical ventilation. Three different methods of respiratory physiotherapy were used - inspiratory muscle training with the Threshold device, positive expiratory pressure device Acapella for facilitation of expectoration and common manual chest physiotherapy. Another objective of the research was to evaluate the way these methods of respiratory physiotherapy will affect the values of maximal inspiratory and expiratory pressures, which are indicative of respiratory muscle strength. The research was conducted at the Department of Anaesthesiology, Resuscitation and Intensive Medicine, University Hospital Ostrava. Mechanical ventilation for at least 48 hours, initiated weaning process, indication for respiratory physiotherapy by the physician and the cooperation of the patient were the inclusion criteria for the patients in the research. A total of 14 patients were enrolled in the research, but only 9 of them completed it. There were 5 female and 4 male patients of average age 70 ? 6.5 years. The average duration of mechanical ventilation before weaning was 19.8 days and the average duration of performed respiratory physiotherapy was 10.6 days. Micro RPM (MD Spiro-Micro Direct, Lewiston, USA) device was used to measure maximal inspiratory and expiratory pressures through the patient's tracheostomy tube. The Kruskal-Wallis test and the T-test for were used for the statistical analysis of the collected data. The results of the research show that there was no statistically significant difference in the effect of the investigated methods of respiratory physiotherapy on the length or outcome of weaning. However, inspiratory muscle training with the Threshold device has led to a statistically significant improvement of maximal inspiratory pressure and thus to an increase of the inspiratory muscle strength. In manual chest physiotherapy group, there was a statistically significant increase of maximal expiratory pressures showing an increase of expiratory muscle strength.
