The role of endoscopic vein harvest in open surgical revascularization of patients with peripheral artery disease

Abstract:

Moderní medicína 21. století se nadále snaží skloubit 2 základní atributy poskytování zdravotní péče. Na jedné straně snaha o vyléčení, nápravu nebo minimálně zmírnění patologických stavů u pacienta. Na straně druhé, poskytnutí této péče s co nejmenší invazivitou pro pacienta. Skloubení těchto 2 protichůdných procesů stálo za zrodem perkutánních endovaskulárních intervencí na počátku 60. let minulého sto-letí, které nabídly řešení stenóz a uzávěrů tepenného systému z drobné perkutánní punkce tepny. I otevřená cévní chirurgie hledala a hledá cesty, které by umožnily provádění cévně-chirurgických operací méně invazivním způsobem. Výsledkem těchto snah bylo zavedení, ale jenom v limitované míře plošné etablování laparoskopických a robotických cévních operací v oblasti intratorakální a aortoiliakální cévní chirurgie. Infrainguinální cévní chirurgie, hlavně v situacích vyžadujících provedení bypassové operace, nadále hledá svoji méně invazivní cestu. Invazivita infrainguinálních bypassových operací je dána hlavně nutností provedení dlouhé incize za účelem odebrání žilního štěpu pacienta, který i v éře protetických, bioprotetických nebo allogenních cévních materiálů nadále představuje „zlatý standard“, tedy materiál s nejlepšími časnými i dlouhodobými výsledky. Jako alternativa ke klasickému otevřenému odběru žilního štěpu se od poloviny 90. let minulého století začínají objevovat práce popisující možnost endoskopického odběru žíly (EVH = endoscopic vein harvest). Nespornou výhodou uvedené metody je možnost provedení kompletního odběru žilního štěpu z jedné krátké cca 3 cm incize za použití specifického endoskopického instrumentária a insuflace podkoží pomocí oxidu uhličitého. Uvedený postup je miniinvazivní, s jednoznačnou redukcí rozsahu kožních incizí na končetině a limitací incidence chirurgických ranných infekcí. To jsou závěry, které soudobé i starší práce popisující využití EVH shodně uvádějí. Specifickou otázkou, kterou metodika EVH přináší, je míra traumatizace žilního graftu v průběhu endoskopického odběru a její komparace s klasickým otevřeným odběrem žilního štěpu. Zde dostupné práce přinášejí nejednoznačné výsledky a to od optimistických až po krajně pesimistické. Je jenom logické, že jasné pochopení vlivu EVH na kvalitu graftu bude stěžejní pro širší etablování uvedené metody. Míra traumatizace žilního graftu v průběhu odběru je faktor, který se zásadně podepisuje na výsledcích infrainguinálních bypassových operací. Cílem práce bylo vyhodnocení vlivu endoskopického odběru na kvalitu žilního graftu, jeho histopatologickou strukturu a výsledky průchodností takto provedených rekonstrukcí. Z toho důvodu bylo provedeno porovnání strukturálních změn stěny otevřeně odebraného a endoskopicky odebraného žilního graftu (vena saphena magna). Z hlediska vyhodnocení dopadu endoskopického odběru na průchodnost cévních rekonstrukcí byl vytvořen soubor pacientů po implantaci infrainguinálního bypassu ve femoropopliteální oblasti. Uvedený soubor umožňuje přímé srovnání průchodnosti infrainguinálních bypassů mezi otevřeně a endoskopicky odebranou véna saféna magna (VSM). Dlouhý interval sledování pacientů uvedeného souboru umožnil srovnání časných, ale i dlouhodobých výsledků implementace metodiky endoskopického odběru VSM.

Abstract:

Modern medicine in the 21st century is endeavouring to combine two basic aspects of health care. On one side, there is an effort to cure and improve, or at least to minimize, the impact of disease states on patients. On the other side, there is a trend to deliver those therapeutical efforts with the least possible invasiveness for the patient. A combination of these two differing aspects resulted in the invention of endovascular interventions at the beginning of the 1960s. Endovascular interventions offer therapy for vascular abnormalities through small percutaneous punctures of a blood vessel, either artery or vein. Open vascular surgery has also been attempting to develop techniques to perform open vascular procedures in less invasive ways. These efforts have resulted in the introduction of laparoscopic and robotic techniques to thoracic and aortoiliac vascular surgeries, although they have not been broadly adopted. Infrainguinal vascular surgery is also attempting to develop less invasive approaches. The invasiveness of infrainguinal bypass operations is based on the need to harvest a long saphenous vein graft from an extremity. Even though there are many prosthetic and allogenic alternatives to the saphenous vein graft, the autologous great saphenous vein is still considered the best conduit for an infrainguinal bypass. None of the alternatives can match the saphenous graft in terms of bypass patencies and their resistance against infection. A great saphenous vein is traditionally harvested in an open manner (open vein harvest). Endoscopic vein harvest (EVH) was invented as an alternative to open vein harvest (OVH) at the end of the 20th century, when the first papers describing the use of the technique were published. An undisputed advantage of EVH is its ability to harvest the whole length of a great saphenous vein through one 3 cm long skin incision with the help of special instruments and carbon dioxide insufflation of the subcutaneous space. This approach is minimally invasive, reduces the length of the vein harvest incision, and results in a reduction of surgical site infections/complications that we otherwise frequently see in infrainguinal bypass surgery. These conclusions are a common theme of both older and more current papers that describe its application. As the method of EVH is completely different from open vein harvest, there have always been questions about the possibility of increased traumatization of the graft. Papers published on the topic have reported contradictory results. Some, especially older studies, are optimistic and describe no harm to the graft; newer studies point to a decreased rate of primary and assisted primary patencies for these grafts. Establishing the extent of trauma to an endoscopically harvested graft is crucial in the broader adoption of this technique. The extent of graft trauma during EVH has the most significant impact on the results of infrainguinal bypass operations. The goal of the project was to establish and compare the extent of graft trauma during EVH and OVH using histopathological examinations of vein samples harvested in both open and endoscopic fashion. Femoral-to-popliteal artery bypasses done using either EVH and OVH of a graft were further compared during the early postoperative period and in the long term. We analyzed the impact of the harvest method on wound complications in the early postoperative period and then on bypass patencies in the long term. As the follow-up interval for both groups was long, we were able to generate a broad range of data on this topic, and this data constitutes the cornerstone of this project.
