Use of cognitive training in patients with quadriplegia

Abstract:

Cílem diplomové práce bylo prozkoumat možnosti využití kognitivního tréninku u pacientů s kvadruplegií. Tento fenomén byl zkoumán pouze u jedné kvadruplegické pacientky (n=1) s posttraumatickým kognitivním deficitem. Tato neurorehabilitace trvala 18 měsíců. Jedná se tedy o kazuistickou studii založenou na kvalitativním výzkumu, ve kterém bylo postupováno pomocí Zakotvené teorie. Výsledky kvantitativních šetření prokázaly až dvojnásobné zlepšení ve všech sledovaných kognitivních funkcích (řečové dovednosti, exekutivní funkce, paměť, pozornost, vnímání). Subjektivní názor pacientky, její rodiny a zdravotnického personálu na neurorehabilitaci kognitivním tréninkem byl posouzen pomocí dotazníku vytvořeného speciálně pro tento výzkum. Odpovědi byly podrobeny obsahové analýze a bylo zjištěno, že pro kvadruplegického pacienta je největším přínosem možnost zvažovat a plánovat svoji další budoucnost a zlepšení se ve vyjadřování potřeb. Další pozitiva byla vnímána v odreagování, posílení self-efficacy a vyslechnutí pacienta. Metoda kognitivního tréninku se tak osvědčila u primárně somatického onemocnění jako je kvadruplegie.

Abstract:

The aim of the thesis was to investigate possibilities of cognitive training in patients with quadrilplegia. This phenomenon was explored only in one quadrilplegic patient (n=1) with posttraumatic cognitive deficit. This neurorehabilitation took 18 months. This is a case study based on a qualitative research realized by the Grounded theory. The results of quantitative methods showed almost double improvement in all the cognitive functions (verbal skills, executive functions, memory, attention, perception). A specifically designed questionnaire evaluated patient´s as well as her family´s and medical staff´s opinions on neurorehabilitation by cognitive training. The responses had been analysed by a concentual analysis and it was concluded that the most appreciated is an opportunity to consider and plan patient’s future and an improvement in expressing needs. The other benefits were an abreaction, increase of self-efficacy and listening to the patient. Cognitive training method has therefore proved itself in primary somatic disease such as quadriplegia.
