Physiotherapy as part of the primary prevention in ischemic heart disease

Abstract:

Cíl práce: Cílem práce je přispět k řešení problematiky volby fyzioterapeutických postupů v rámci primární prevence u ischemické choroby srdeční. Metodika: V této práci byl použit rozhovor pro sestavení anamnézy, pozorování, měření a testování k sestavení vstupního a výstupního kineziologického rozboru. Na základě výsledků vstupního kineziologického rozboru byl navržen krátkodobý fyzioterapeutický plán na 12 týdnů. V každém týdnu bylo jedno vedené fyzioterapeutické sezení a dva dny pacientka cvičila sama na základě navrženého plánu. Při terapii byly využity techniky měkkých tkání, míčková facilitace, postizometrická relaxace a postfacilitační strečink, metoda Ludmily Mojžíšové, Brügger koncept a senzomotorická stimulace. Výsledky: Výstupní kineziologický rozbor prokázal zlepšení fyzické kondice pacientky. Oproti vstupnímu vyšetření zvládla ujít o 100 m více a s výsledkem 600 ujitých metrů se zařadila do kategorie, kde fyzická zdatnost odpovídá věku. Tento výsledek byl potvrzen i kontrolní zátěžovou ergometrií. Pacientce se zvýšila tolerance zátěže ze 150 W na 200 W a test trval o 2 minuty déle než před fyzioterapií. Došlo také k úbytku váhy, pacientka zhubla 7 kg. Výpočet BMI ukázal hodnotu 31,63, tím se pacientka přiblížila více ke spodní hranici mezi nadváhou a obezitou I. stupně. Vyšetření trofiky prokázalo úbytek váhy zmenšením obvodů v problémových oblastech, nejvíce se obvody zmenšily v pase, přes pupík a přes boky.

Abstract:

Objective: The goal of this work is to assist with choise of the physiotherapy procedures related to primary prevention of ischemis heart disease. Methodics: In this thesis were used methods as conversation to create anamnesis, observation, measuring and testing to make entrance and final kinesiological examination. On the basis of entrance exam was arranged short-term physiotherapy program for 12 weeks. Each week contained one guided physiotherapy session and for two days patient exercise on her own. In therapy was used techniques of soft tisssues, facilitation with silk ball, post-isometric relaxation and post-facilitation stretch, method of Ludmila Mojžíšová, Brügger's concept and sensorimotor stimulation. Results: Final kinesiological anamnesis proved improvement of patient's physical condition. She managed to walk about 100 metres more than in the entrance exam and her result 600 metres is in the chart in category, where the distance responds to her age. This findings was confirmed by control ergometry exam. Her tolerance of physical stress improved from 150 W to 200 W and testing lasted 2 minutes longer than before physiotherapy. She also lost 7 kilograms of her wieght. BMI calculation showed number 31,63, that is closer to overweight than first grade of obesity. Measurements of problematic body parts proved loosing weight, greatest improvement was at waist, hips and through the navel.
