The influence of lifelong rehabilitation at morbus Bechterev

Abstract:

Úvod: Tato bakalářská práce se zabývá řešením problematiky celoživotní fyzioterapie u pacienta s morbus Bechtěrev. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s morbus Bechtěrev s využitím vhodných fyzioterapeutických konceptů a metod. Metodika: V hlavní části jsem zpracovala kazuistiku pacienta s onemocněním morbus Bechtěrev V. stupeň. Na základě výsledků ze vstupního kineziologického rozboru (vyšetření pohyblivosti páteře, mobility hrudníku, goniometrie) jsem sestavila krátkodobý fyzioterapeutický plán s využitím metod a konceptů fyzioterapie (PNF, DNS, PIR, mobilizační techniky, techniky měkkých tkání, respirační fyzioterapie, koncept dle Brüggera, SM - systém). Fyzioterapeutická intervence trvala 3 měsíce, terapie se uskutečnily jednou týdně. V polovině fyzioterapeutické intervence (6. týden) jsem provedla kontrolní vyšetření (vyšetření pohyblivosti páteře, mobility hrudníku a goniometrie krční páteře). Výsledky: Došlo ke zlepšení posunlivosti kůže, podkoží a fascií v oblasti zad a C-Th přechodu. Byla zvětšena hybnosti hrudníku o 3 cm. Dále se zvětšil rozsahu pohybu v krční páteři do lateroflexe 5° aktivně i pasivně, vpravo i vlevo. Rotace se zvětšila o 5° aktivně i pasivně, vlevo i vpravo. U vyšetření pohyblivosti páteře došlo ke zvětšení rozsahu pohybu u zkoušky lateroflexe o 2 cm vlevo a 3 cm vpravo. Ottův inklinační index se zvětšil o 0,5 cm a Ottův reklinační index se zmenšil o 0,5 cm. Čepojův příznak se zvětšil o 1 cm a došlo k zmenšení o 1 cm u Thomayerova příznaku. Závěry: Pro pacienty s morbus Bechtěrev je pohyb stěžejní prostředek léčby tohoto onemocnění. Komplexní fyzioterapeutickou péčí s využitím vhodných konceptů a metod můžeme ovlivnit a zlepšit pacientům kvalitu jejich života. Poznatky z této práci jsou užitečné pro mou nadcházející praxi.

Abstract:

Introduction: This bachelor thesis deals with solving the problem of lifelong physiotherapy in a patient with Bechterev's disease. The objective: The aim of the bachelor thesis is to contribute to the solution of physiotherapy in patients with Bechterev's disease using appropriate physiotherapeutic concepts and methods. Methodology: In the main part I processed a case report of a patient with Bechterev's disease V. degree. Based on the results of the initial kinesiological analysis (examination of spinal mobility, chest mobility, goniometry), I compiled a short-term physiotherapy plan using physiotherapy methods and concepts (PNF, DNS, PIR, mobilization techniques, soft tissue techniques, respiratory physiotherapy, Bruügger, SM concept - system). The physiotherapeutic intervention lasted 3 months, the therapies were performed once a week. In the middle of the physiotherapeutic intervention (week 6) I performed a control examination (examination of spinal mobility, chest mobility and goniometry of the cervical spine). Results: There was an improvement in the displacement of the skin, subcutaneous tissue and fascia in the area of the back and C-Th junction. The momentum of the chest increased by 3 cm. There was an increase in the range of motion in the cervical spine to a lateroflex of 5 ° actively and passively, right and left. The rotation increased by 5 ° both actively and passively, on the left and on the right. In the examination of spinal motility, the range of motion increased in the lateroflex test by 2 cm on the left and 3 cm on the right. Otto's inclination index increased by 0.5 cm and Otto's inclination index decreased by 0.5 cm. Cepoj's flag increased by 1 cm and decreased by 1 cm in Thomayer's flag. Conclusions: For patients with Bechterev's disease, exercise is a key means of treating the disease. Comprehensive physiotherapeutic care using appropriate concepts and methods can influence and improve patients' quality of life. The findings from this work are certainly useful for my upcoming practice.
