Abstract:

Úvod: Tématem mé bakalářské práce je fyzioterapie u klientky s poporodní diastázou mm. recti abdominis. Seznámením se s možnostmi ovlivnění vzniklé diastázy pomocí fyzioterapie a poznatky s tím spojenými, jsem se snažila o vytvoření uceleného pohledu na danou problematiku a tím přispět k jejímu řešení. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie při poporodní diastáze mm. recti abdominis. Metodika: Hlavní část mé bakalářské práce popisuje kazuistiku pacientky s poporodní diastázou mm. recti abdominis. s klientkou jsem spolupracovala 4 měsíce a proběhlo 15 intervencí. Při první fyzioterapeutické intervenci jsem provedla vstupní kineziologické vyšetření s využitím pozorování, testování a měření (vyšetření aspekcí a palpací, vyšetření hybnosti, antropometrie břicha, hrudníku a pánve, vyšetření zkrácených struktur a pohybových stereotypů, vyšetření hypermobility a trupové stabilizace), na jehož základě jsem vypracovala krátkodobý rehabilitační plán. V průběhu naší spolupráce jsem provedla kontrolní vyšetření během páté intervence. Obsahem cvičebních jednotek bylo využití fyzioterapeutických konceptů a metod (respirační fyzioterapie, techniky měkkých tkání, PIR, DNS, ACT ?, cvičení s využitím pomůcek a jiné) k ošetření měkkých tkání, nácviku správného dechového stereotypu a dynamiky dýchání za účelem ovlivnění správného zapojení svalů hlubokého stabilizačního systému páteře. Během poslední intervence jsem vytvořila výstupní kineziologický rozbor a na jeho základě dlouhodobý fyzioterapeutický plán. Výsledky: U pacientky došlo ke zlepšení stavu měkkých tkání, vadného držení těla, dechového stereotypu, trupové stabilizace a aktivace nitrobřišního tlaku spolu se zmenšením poporodní diastázy mm. recti abdomins v nějširším úseku měření z 3,5 cm na 2,6 cm i přes přítomnost umbilikální kýly. Závěry: Z hlediska pozitivních výsledků po skončení mé spolupráce s pacientkou se způsob terapie zvolený během intervencí prokazuje být účinný.

Abstract:

Introduction: The topic of my bachelor thesis is physiotherapy for a client with postpartum diastasis mm. recti abdominis. By studying the possibilities of influencing the resulting diastasis using physiotherapy and the knowledge associated with it, I tried to create a comprehensive view of the issue and thereby contribute to its solution. The goal of the thesis: The goal of this bachelor thesis is to contribute to solving the issue of physiotherapy in postpartum diastasis m. rectus abdominis. Methodology: The main part of my bachelor thesis describes the case study of a patient with postpartum diastasis mm. recti abdominis. I worked with the client for 4 months and 15 interventions were performed. During the first physiotherapy intervention, I performed an initial kinesiological examination using observation, testing and measurement (examination of aspection and palpation, examination of mobility, anthropometry of the abdomen, chest and pelvis, examination of shortened structures and movement stereotypes, examination of hypermobility and trunk stabilization), because of which I developed a short-term rehabilitation plan. During our cooperation, I performed a follow-up examination during the fifth intervention. The content of the exercise units was the use of physiotherapeutic concepts and methods (respiratory physiotherapy, soft tissue techniques, PIR, DNS, ACT ?, exercises with the use of aids and others) to treat soft tissues, practice the correct breathing stereotype and breathing dynamics to influence the correct involvement of the muscles of the deep stabilization system. Results: The patient experienced an improvement in the condition of soft tissues, poor posture, breathing stereotype, trunk stabilization and activation of intra-abdominal pressure together with a reduction in the postpartum diastasis of mm. recti abdomins in the widest section of the measurement from 3.5 cm to 2.6 cm despite the presence of umbilical hernia. Conclusions: In terms of positive outcomes at the end of my collaboration with the patient, the method of therapy chosen during the interventions proves to be effective.
