Indexes of insulin resistence and compared with methods of indirect determination of insulin sensitivity

Abstract:

Diplomová práce se zabývá problematikou inzulínové rezistence a možnostmi její diagnostiky. Cílem práce bylo srovnání určení inzulínové rezistence pomocí homeostatických indexů a jejich nepřímých ukazatelů a následné vyjádření korelace mezi těmito metodami. Teoretická část práce pojednává o problematice inzulinorezistence, jejich rizikových faktorem, technikách diagnostiky, možnostech léčby a prevence. Dále jsou v práci uvedeny nemoci, které se s inzulínovou rezistencí úzce pojí, a těmi jsou metabolický syndrom a diabetes mellitus. Praktická část je zpracována statistickou metodou regresní analýzy, kde na základě databáze poskytnuté prof. Martiníkem byla vybrána skupina pacientů, kterým byla provedena vyšetření potřebná pro analýzu. Na základě hodnot inzulínu na lačno a glykémie na lačno byly vypočteny homeostatické indexy QUICKI a HOMA IR a následně byla zjišťována jejich korelace s laboratorními ukazateli stanovovanými při diagnostice metabolických poruch. Výsledky prokázaly silnou korelaci mezi homeostatickými indexy a lačným inzulínem a mírnou korelaci indexů s triglyceridy. Ukazatele triglyceridémie a inzulinémie se považují za přímé metody stanovení inzulínové rezistence. HDL cholesterol a lačná glykémie nejsou typickými metodami průkazu inzulínové senzitivity a jejich korelace s HOMA a QUICKI indexy byla slabá nebo dokonce žádná.

Abstract:

The thesis deals with the problems of insulin resistance and its diagnostics options. The aim of the thesis was to compare the determination of insulin resistance using homeostatic indexes and their indirect indicators and the expression of correlation between these methods. The theoretical part is about issues of insulin resistance, their risk factors, diagnostic techniques, treatment options, and about prevention. Furthermore, the work presented diseases that with insulin resistance are in close relationship, and those are the metabolic syndrome and diabetes mellitus. The practical part is statistical method of regression analysis, which based on a database provided by prof. Martiník, has been selected group of patients who underwent required examinations important for the analysis. Based on the blues of fasting plasma insulin and fasting plasma glucose were calculated homeostatic indexes QUICKI and HOMA, and then was research their correlation with laboratory markers to be determined in the diagnosis of metabolic disorders. The results showed a strong correlation between indexes of insulin resistance and fasting plasma insulin and a moderate correlation of triglycerides and the indexes. Triglyceridemia and insulinemia are considered as a indirect method of determining insulin resistance. HDL cholesterol and fasting plasma glucose levels are not typical methods for the detection of insulin sensitivity and their correlation with HOMA and QUICKI indexes are weak or even no.
