Initial Down Syndrome Diagnosis: Communication with Mothers

Abstract:

Práce se zabývá problematikou sdělování prenatální a raně postnatální diagnózy Downova syndromu matkám postižených dětí. Teoretická část práce je zaměřena především na popsání faktorů, které do procesu vstupují jednak na straně rodičů a jednak na straně sdělujících lékařů. Je v ní zdůrazněno, jaký význam má okamžik sdělení v následném procesu akceptace nepříznivé informace a přijetí dítěte. Dále jsou uvedena doporučení, která lékařům dávají autoři zabývající se tematikou sdělování. Výzkumná část práce si klade za cíl zmapovat okamžik sdělení z pohledu matek, které jsou příjemkyněmi tohoto sdělení. Výsledkem výzkumné části je přehled témat, která jsou pro matky, v souvislosti se sdělením, důležitá. Doporučení, která na základě své zkušenosti dávají matky lékařům, se v mnohém shodují s doporučeními prezentovanými v teoretické části práce. Jako ústřední téma okamžiku sdělení se ukazuje vlastní iniciativa matek. Ta měla v případě respondentek často vliv na přístup lékařů, množství získaných informací a tedy i péči dostupnou pro dítě. Závěrem práce je zhodnocení výzkumu a doporučení lékařům, aby sami v procesu sdělení diagnózy převzali iniciativu.

Abstract:

Initial Down syndrome diagnosis: Communication with mothers. Theoretical part of the study presents overview of variables influencing the moment of delivering prenatal and postnatal diagnosis. It describes importance of the manner how the diagnosis is delivered and its influence on the process of acceptation this information and following adaptation on a new life with children. It also includes the list of recommendations concerning the process of communication of unfavorable information that are given to health professionals. The empirical part of the study present research of mothers perceptions of the delivering diagnosis of their children. It refers to different accents in this process. Mothers were also asked to summarize their own experiences in recommendations to physicians which could help them to manage communication of Down syndrome diagnosis better. These advices were compared to recommendations in the theoretical part of study. Analysis of interviews with mothers has shown key theme - initiative of mothers. It influences approach of physicians and also the amount of information that are given to parents and consequently the care of the child. The conclusion evaluates the study and emphasizes the recommendation for physicians to undertake the initiative in process of delivering diagnosis.
