Physiotherapy after ACL operation and gait instrumental evaluation

Abstract:

Název: fyzioterapie po operaci LCA (ligamentum cruciatum anterius) a přístrojové ohodnocení chůze. Cíl práce: přispět k řešení problematiky fyzioterapie po operaci LCA (po dobu 6 týdnů 1 - 2krát týdně a následně až do období 17. týdne od operace). Metodika: v rámci vstupního kineziologického rozboru aspekcí a palpací zhodnotit držení těla, konfiguraci, trofiku, tonus a hybnost, pomocí svalového testu dle Jandy ozřejmit svalovou sílu, pomocí antropometrického měření stanovit délky a obvody dolních končetin a měřením goniometrickým zjistit rozsahy pohybů v kloubech dolních končetin. Dále v rámci manuálních technik zhodnotit aktuální stav měkkých tkání včetně pooperačních jizev. Sledovat stereotyp chůze včetně chůze po schodech a stabilitu kolenního kloubu při chůzi. Výsledky: v pooperačním období byly zjištěny patologie měkkých tkání, blokáda hlavičky fibuly a pately, snížený ROM (range of motion) levého kolenního kloubu a svalové síly, nestabilita levého kolenního kloubu a díky odlehčené chůzi pomocí francouzských holí nesprávný stereotyp chůze. Na základě výsledků vyšetření byly využity v terapii tyto prostředky: techniky měkkých tkání, mobilizace pately a hlavičky fibuly, aktivní cvičení i proti odporu pro zvětšení rozsahu pohybu s pomůckami (overball, fyzioball, thera-band, rotoped atd.). Dále byla využita technika postizometrické relaxace, postfacilitačního strečinku, rytmické stabilizace, reciproční inhibice a pro zvýšení svalové síly metoda založena na neurofyziologickém podkladě PNF (proprioceptivní neuromuskulární facilitace). Závěr: vlivem intenzivní fyzioterapeutické intervence v období do 17. týdne od operace bylo dosaženo zahojení pooperačních jizev, snížení otoku kolenního kloubu, získání plného ROM kolenního kloubu, zvýšení svalové síly svalů kolenního kloubu a zlepšení stability kolenního kloubu levé dolní končetiny. V souvislosti s tím byl obnoven stereotyp chůze a došlo ke zlepšení celkového držení celého těla.

Abstract:

Title: Physiotherapy after LCA (ligamentum cruciatum anterius) surgery and instrumental evaluation of walk. The aim of the work: To solve physiotherapy problem after LCA surgery (for 6 weeks 1-2 times per week and then up to the 17th week after operation). Methodology: Within the input kinesiology analysis of aspection and palpation to evaluate posture of body, configuration, trophic, tone and momentum. Using the Jandas's muscle test to clarify muscle strength. With anthropometric measurements determine the length and circuits of the lower limbs and through trigonometric measurement determine the range of motion in the joints of lower limbs. Furthermore, in the context of manual techniques to assess the current status of soft tissues including post-surgical scars. Follow the stereotype of walk, including walking up the stairs and stability of the knee joint during walking. Results: In post-operative period were observed: pathology of soft tissues, blockade of the fibular head and patella, reduction of the left knee (joint) ROM and muscle strength, instability of the left knee joint and thanks to the lightweight walking with the help of forearm crutches incorrect stereotype of walk. Depending on the examination results following methods were used in the therapy: soft tissues techniques, mobilization of patella and fibular head, active exercises against the resistance to increase the range of motion with aids (f. e. overball, physioball, thera-band, bicycle, etc.) Further the techniques of postisometric relaxation, post-facilitation stretching, rhythmic stabilization, reciprocal inhibition and to increase muscle strength the method based on the neurophysiological basis of PNF (proprioceptive neuromuscular facilitation) were used. Conclusion: We achieved thanks to intensive physiotherapy intervention in at period till 17th week after the operation following: healing of post-operative scars, reduction in the knee joint swelling, obtaining full ROM of the knee joint, increase muscle strength of muscles of knee joint and improve the stability of knee joint of left leg. In connection with this, stereotype of walk was restored and overall posture of the body was improved.
