Subjective Experiencing of a Disease by Children with Selective Gastroenterological Ilnesses

Abstract:

Cíl: Hlavním cílem diplomové práce bylo zjistit a popsat prožívání nemoci u dětí s vybranými gastroenterologickými onemocněními. Dílčím cílem bylo popsat prožívání nemoci a pocity dětských pacientů od období stanovení diagnózy až po současnost. Metodika: K dosažení cíle výzkumu byla použita metoda kvalitativního výzkumného šetření. Klíčovou metodou sběru dat byl polostrukturovaný rozhovor vedený s dítětem, které trpí Crohnovou nemocí, ulcerózní kolitidou či celiakií, dále byla použita metoda pozorování a studium dokumentace. Prostřednictvím doslovné transkripce byly audiozáznamy jednotlivých rozhovorů přepsány do textové podoby a získané výpovědi dětí (n=15) byly podrobeny obsahové analýze. Pomocí analýzy a otevřeného kódování byly získané odpovědi rozděleny do výsledných řešených oblastí. 1. Diagnostika nemoci, 2. Prožívání nemoci v oblasti fyzické, 3. Prožívání nemoci v oblasti psychické, 4. Prožívání nemoci v oblasti sociální. Výsledky: Výsledkem našeho výzkumu je zjištění, že nejvýznamnější problém v oblasti fyzické představovala pro děti změna stravovacích návyků a omezení fyzické aktivity. Z výpovědí dětí je taktéž zřejmé, že chronická nemoc zanechala významné stopy i na jejich psychice, jelikož pro všechny účastníky nebylo jednoduché se s chronickou nemocí vyrovnat. Nemocí došlo k ovlivnění i sociální stránky takto nemocných dětí, protože zpětné zařazení do kolektivu pro některé nebylo vůbec snadné. Závěr: Závěrem lze říci, že u všech dětí se výpovědi shodovaly a vybraná gastroenterologická onemocnění zasáhla jak fyzickou, psychickou, tak sociální oblast dispenzarizovaných dětských pacientů. Závěry tohoto kvalitativního výzkumného šetření poskytují možnost pochopit problematiku vybraných gastroenterologických onemocnění u dětí a docílit tím zlepšení ošetřovatelské péče a psychické a sociální podpory dětskou sestrou.

Abstract:

Aim: The main aim of the thesis was to identify and describe the experience of illness in children with selected gastroenterology diseases. A secondary aim was to describe the illness experience and feeling sof children from the period of diagnosis until the present. Methodology: Qualitative research was used in our thesis. The key method of data collection was semi/structured interview conducted with a child who has Crohn´s disease, Ulcerative colitis or Celiac disease also was used the method of observation and studying the documentation. The audiorecordings of individual interview were transcribed through a transcription into text form and obtained answers of children (n=15) were subjected to kontent analysis then were dividend into the resulting solved areas through the analysis and open coding. 1. Diagnosis of the disease, 2. The experiences with illness in physiacal areas, 3. The experiences with illness in mental areas, 4. The experiences with illness in social areas. Results: In our thesis we found that the most important problem in physical areas is changing eating habits and limitations of physical aktivity. The children also said that the chronic disease had influence to thein mental health and for all of participants was not easy to accept thein chronic disease. The chronic disease had also influence to social areas of children because returning to group of children was very difficult for them. Conclusion: In conclusion we can say that the response sof all children were the same and selected gastroenterology diseases had influence especially to physical, mental and social areas of children. The conclusions of our qualitative research provils an oportunity to understand this gastroenterology diseases in children and also achieve the improvement of nursing care and psychological and social support pediatric nurse.
