The physioterapy of clients with progressive lateral sclerosis

Abstract:

Cíl: Cílem mé práce je přispět k řešení problematiky výběru vhodných fyzioterapeutických postupů, konceptů a metod u vybraného pacienta s progresivní laterální sklerózou. Metodika: Provedla jsem vstupní kineziologický rozbor, základní antropometrické a orientační goniometrické vyšetření pomocí krejčovského metru a goniometru, vyšetřila jsem funkční svalovou sílu dle Jandy. Aspekcí a palpací jsem zhodnotila postavení pacienta, trofiku a chůzi. Následně jsem provedla neurologické vyšetření fyzioterapeutem fyziologické a patologické reflexy pomocí neurologického kladívka, hodnocení soběstačnosti podle Barthelova testu ADL, Rombergův test stability a posouzení míry spasticity dle Ashworthovy škály. Na základě kineziologického rozboru a anamnézy jsem stanovila krátkodobý rehabilitační plán na dobu šesti měsíců s využitím fyzioterapeutických postupů, konceptů. Použila jsem míčkovou facilitaci dle Jebavé, respirační fyzioterapii, pasivní a aktivní pohyb, MT a PIR, dále koncepty PNF, Spiraldynamic, Sensomotorickou stimulaci, Bobath koncept a fyzioterapeutické pomůcky jako je overball a TheraBand. Po výstupním vyšetření jsem navrhla dlouhodobý rehabilitační plán. Výsledky: Po terapii se klientce zlepšilo dýchání, především do hrudní oblasti, došlo ke zvýšení rozsahu pohybu v ramenním kloubu do flexe o 40 stupňů, do abdukce o 30 stupňů, v kyčelním kloubu do flexe o 10 stupňů a v hlezenním kloubu do dorsální flexe o 10 stupňů. Klientce se zvýšila stabilita, jak v sedě, tak i ve stoje, dle Rombergova testu o jeden stupeň. Vyšetření spasticity dle Ashworthovy škály prokázalo zlepšení o dva stup-ně na akrech HKK a na první stupeň svaly, vykonávající addukci LDK. Snížila se bolestivost Lp a SI kloubů. Došlo k malému zvýšení svalové síly, pacientka zvládne pohyb proti lehkému odporu a k výraznému zlepšení chůze. Klientka začala chodit o berlích.

Abstract:

Aim of work: The objective of the thesis is a contribution of solving the problems of selecting appropriate physiotherapeutic procedures, concepts and methods in selected pa-tient with progressive lateral sclerosis. Methodology: I conducted an incoming kinesiology analysis, the basic anthropometric and tentative goniometric examination with a sartorial metr and goniometr. I invastigated the funcional muscle strength which was improve according to Janda test. With the help of aspection and palpation I evaluated the posture of the patient, his trophic and walking. Then I performed a neurological examination by a physiotherapist physiological and pat-hological reflexes with the neurological hammer, evaluation of self-sufficienty by Barthel test ADL, Romberg test of the stability and asses the degree of spasticity by Ashworth scale. Based on the kinesiology analysis I set short term rehabilitation plan for six month using physiotherapy techniques and concepts. I used to facilitation with the small ball by Jebavá, the respirátory physiotherapy, pasive and active movement, MT, PIR, then I used PNF, Spiraldynamic?, Sensomotircal stimulation, Bobath concept and physiotherapeutic aids like an overball and TheraBand. After the final examination I proposed long-term plan. Results: After therapy, the client improve breathing, specially in the chest area, increased range of motion in the schoulder joint of 40 degrees to the flexion, about 30 degrees to the abduction, in the hip joint of 10 degrees to the flexion and in the ancle joint of 10 degrees to he flexion. The client improved stability in a sitting and standing position by Romberg test of 1 segree. Examination of the spasticity by Ashworth scales show improving of 2 degrees on acre of upper limbs and at first level goes the left lower limb. The pain of the lumbal spine and SI joints comes down. The muscle strength was also slightly better, the patient handle movement againts a litlle resistence and walking is better too. The client star-ted to walk with a crutches.
