Pevnost vazby ortodontických adhezivních materiálů a jejich vliv na sklovinu

Abstract:

Úvod: Pro lepení ortodontických zámků máme k dispozici mnoho různých přípravků. Pokud chceme správným a vhodným způsobem nalepit součásti fixního aparátu, potřebujeme znát, jaká je pevnost vazby jednotlivých adhezivních materiálů, jaké je chování vrstvy adheziva při zátěži a také, zda při použití ortodontických adhezivních materiálů nedochází k poškození skloviny. Cíle práce: Stanovit pevnost vazby ortodontických adhezivních materiálů jak na sklovinném, tak na umělém povrchu. S tím souvisí také vyhodnocení tzv. ARI (Adhezive Remnant Index), který vypovídá o tom, kde v „adhezivní“ oblasti dochází k porušení vazby. Dále potom bylo cílem práce prošetřit, zda při použití daných ortodontických adhezivních materiálů nedochází k poškození sklovinného povrchu. Materiál a metodika: Do studie bylo zařazeno 6 adhezivních materiálů – ze skupiny skloionomerních cementů GC Fuji ORTHOTM a GC Fuji ORTHOTM LC, kompozitní materiály TransbondTM XT, NeobondTM a BrackFix NT a akrylátová pryskyřice Spofacryl. Při stanovování pevnosti vazby bylo vyhodnoceno celkem 156 měření na povrchu extrahovaných premolárů, 120 měření na povrchu kompozitním a pryskyřičném a 90 zkoušek na povrchu keramickém. ARI a množství prasklin skloviny byly proměřen na fotografiích vzorků extrahovaných premolárů. Výsledky byly následně statisticky vyhodnoceny. Výsledky: Nejlepších výsledků pevnosti vazby dosahoval na sklovinném, kompozitním a keramickém povrchu chemicky tuhnoucí kompozitní materiál BrackFix NT. Na pryskyřičném povrchu byla změřena nejvyšší pevnost vazby u materiálu Spofacryl. Výsledky ARI potvrdily u skloionomerních cementů větší zůstatky lepidla na bázi zámku, u ostatních materiálů na testovaném povrchu. V některých případech byla zjištěna závislost mezi velikostí pevností vazby a ARI. U žádného testovaného materiálu nebyl zjištěn statisticky významně vyšší výskyt prasklin skloviny po sejmutí zámku. Závěr: Podařilo se splnit všechny cíle práce a stanovit jejich význam pro praxi a pro oblast výzkumu a vývoje nových adhezivních materiálů. Práce byla podporována projektem SVC 1M0528.

Abstract:

Introduction: There are many adhesive materials available for bonding of orthodontic brackets. When we want to fix parts of fixed orthodontical appliance correctly, we need to know the bond strength of used materials, the behaviour of the adhesive layer during loading. Another important thing is the occurence of enamel fractures after debonding of the bracket. Aims: To determine the bod strength of orthodontic adhesive materials on the enamel surface of extracted premolars and on artificial surfaces. To evaluate so called ARI (Adhesive Remnant Index), which gives us information about the location af bond failure in the adhesive area. The next aim was to verify, whether during usage of orthodontic adhesive materials the frequency of enamel fractures rises. Material and methods: We included 6 orthodontic adhesives into this study - glassionomer cements GC Fuji ORTHOTM and GC Fuji ORTHOTM LC, composite resin materials TransbondTM XT, NeobondTM and BrackFix NT and acrylic resin Spofacryl. On extracted premolars 156 measurements of bond strength were undertaken, 120 measurements on composite and resin surface and 90 on the ceramic surface. ARI and enamel fractures were determined on photos of extracted premolars specimens after debonding of the orthodontic brackets. All results were statistically evaluated. Results: The highest values of bond strength on enamel, ceramic and composite surface were achieved by material BrackFix NT. On the resin surface it was the acrylic resin Spofacryl. The results of ARI confirmed more remnants of adhesives on the bracket base, while other materials showed more of the adhesive on the tested surface. In some cases there were found dependencies between the bond strength and ARI. No statistically significant increments of the numbers of enamel fractures were detected. This is valid for all of the tested adhesive systems. Conclusions: We succeeded in acquiring results in all of the tasks of this study. We determined the impact of these results either for practices or research of new materials. The study was supported by project SVC 1M0528.
