Physiotherapy for unilateral sports load

Abstract:

Úvod: V mojí bakalářské práci se zaměřuji na fyzioterapii u jednostranné sportovní zátěže - hokejbal. Hokejbal je kontaktní kolektivní sport podobný hokeji. Tím, že se nejedná o profesionální sport, tak hráči nejsou tolik vedeni ke kvalitní rozcvičce, zahřátí a závěrečnému strečinku. To vede k častým zraněním, které hráči běžně léčí nedostatečným klidem, často bez lékařské kontroly. Dochází tím k časté recidivě poranění přecházejícího do chronického stavu. Další z věcí, která se opomíjí, jsou kompenzační cvičení, jejichž vynechávání může vést ke skoliotickému držení těla. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u jednostranné sportovní zátěže pomocí vhodně zvolených fyzioterapeutických postupů, metod nebo konceptů u pacienta. Metodika: Práce byla vytvořena na základě kazuistiky jednoho probanda - 26 let, který je aktivním hráčem extraligy hokejbalu mužů - nejvyšší české soutěže. Na základě vstupního kineziologického rozboru byl navržen krátkodobý fyzioterapeutický plán složený z 10 fyzioterapeutických intervencí, které se uskutečnily během 12 týdnů. V rámci terapií byly použity koncepty PNF, DNS, ACT, Brügger koncept, techniky měkkých tkání, masáže a PIR. V polovině fyzioterapeutického plánu byl proveden kontrolní rozbor pro kontrolu vybraných patologických parametrů. V rámci poslední terapie byl proveden výstupní kineziologický rozbor, na jehož základě byl navrhnut dlouhodobý fyzioterapeutický plán. Výsledky: Pomocí fyzioterapeutických intervencí se podařilo pozitivně ovlivnit ROM do FLEX v kyčelním kloubu, odstranit bolest hamstringů a bederní páteře. Nadále došlo ke zlepšení skoliotického držení páteře. Zlepšila se správná aktivace HSSP a udržení nitrobřišního tlaku ve vyšších posturálních pozicích. Pohybové vzorce EXT kyčle, FLEX šíje a trupu byly upraveny a vedly ke správnému zapojení svalových řetězců při pohybu. Závěry: Bakalářská práce splnila hlavní cíl a úkoly problematiky fyzioterapie u jednostranné sportovní zátěže, objasnila teoretickou analýzu odborné literatury, která shrnuje problematiku dané diagnózy. Byl proveden vstupní a výstupní kineziologický rozbor s vyšetřeními zahrnující problematiku využití fyzioterapie u pacienta, který potvrdil zlepšení zdravotního stavu.

Abstract:

Introduction: The topic of my Bachelor's thesis is physiotherapy of unilateral sport load - street ball hockey. Ball hockey is a contact team sport similar to ice hockey. Due to the fact, that ball hockey is not a professional sport, the players are not led to perform a quality warm-up and post-exercise stretching. This leads to frequent injuries that the players usually treat by insufficient rest, often without a specialized doctor's visit. This ultimately results in relapsing injuries that transition to a chronic state. Furthermore, compensation exercises are also usually omitted and their omission might lead to scoliosis. The objective: The objective of this Bachelor's thesis is to contribute to the solution of physiotherapy problems regarding unilateral sport load using appropriately selected physiotherapeutic procedures, methods, and concepts with the patient. Methodology: This thesis was conceived based on a casuistry of one test subject who is an active street ball hockey player (26 y.o.) in the highest Czech league. Based on the entry kinesiologic examination, a short-term physiotherapeutic plan was designed. This plan consisted of 10 physiotherapeutic interventions during 12 weeks. During the physiotherapeutic treatment, I used concepts PNF, DNS, ACT, Brügger concept, Kaltenborn mobilization exercise method, soft tissue techniques, massage, and PIR. Halfway through the physiotherapy session, a control examination was performed to obtain selected parameters. During the last therapy session, an exit kinesiologic examination was performed, and a long-term physiotherapeutic plan was designed. Results: Using physiotherapy intervention, I managed to positively influence ROM to FLEX in the hip joint and to remedy the hamstring and lower back pain. Furthermore, I partially influenced the patient's scoliotic posture. I recorded an improvement in correct activation of the deep spinal stabilization system and a retaining of intra-abdominal pressure in elevated positions. The motion patterns of the EXT hip, FLEX neck and chest were modified and the correct activation of muscular chains during motion was recorded. Conclusions: This Bachelor's thesis fulfilled its main objectives and tasks regarding the problems of physiotherapy of unilateral sport load and clarified the theoretical analysis of specialized literature. Entry and exit kinesiology testing aimed to examine issues of in-patient physiotherapy usage was performed and an improvement in the patient's health condition was confirmed.
