Physiotherapy after reconstruction of ligamentum cruciatum anterior

Abstract:

Název: Fyzioterapie po plastice předního zkříženého vazu Cíl: Cílem práce je přispět k řešení problematiky vhodného výběru fyzioterapeutických metod, postupů a konceptů u pacienta po plastice předního zkříženého vazu v rámci ambulantní péče. Dalším cílem je zlepšit informovanost široké veřejnosti o fyzioterapii této diagnózy. Metodika: Zpracovala jsem kazuistiku 27 letého pacienta s diagnózou Plastika předního zkříženého vazu kolenního kloubu. Vstupní kineziologický rozbor pacienta, který jsem provedla před operací a následně 14 dní po operaci v rámci zahájení ambulantní léčby, jsem zaměřila na rozsah pohybu a svalovou sílu operované dolní končetiny. Dle získaných výsledků jsem stanovila krátkodobý fyzioterapeutický návrh, který byl následně aplikován během 10 společných fyzioterapeutických intervencí. K terapii byly použity tyto metody: polohování, kryoterapie, cévní a dechová gymnastika, techniky měkkých tkání, míčková facilitace, PFS (postfacilitační strečink), aktivní cvičení s overballem, therabandem, cvičení v uzavřených a otevřených kinematických řetězcích. Během 10 společných terapeutických intervencích jsem prováděla průběžná kontrolní měření rozsahu pohybu a svalové síly operované dolní končetiny. Během poslední terapeutické intervence jsem provedla výstupní vyšetření a na jeho základě sestavila dlouhodobý fyzioterapeutický plán. Výsledky: Na základě provedeného krátkodobého fyzioterapeutického plánu došlo k navrácení plného rozsahu pohybu, zvětšení svalové síly, nápravě stereotypu chůze, odstranění svalového zkrácení a odbourání nestability. Závěr: Díky včasnému a intenzivnímu zapojení fyzioterapeutických metod do pooperační léčby pacienta bylo dosaženo plného rozsahu pohybu, zlepšení svalové síly, rovnováhy a minimalizace možných následků po operaci předního zkříženého vazu kolenního kloubu.

Name: Physiotherapy after reconstruction of the anterior cruciate ligament Aim: The aim of the work is to contribute to the solution of the issue of a suitable selection of physiotherapeutic methods, procedures and concepts in a patient after reconstruction of the anterior cruciate ligament in out-patient care. Another aim is to improve awareness of the general public about physiotherapy of this diagnosis. Methodology: I prepared casuistry of a 27-year-old patient with the diagnosis Reconstruction of the anterior cruciate ligament of the knee joint. In the entry kinesiological analysis of the patient that I carried out before the operation and consequently 14 days after the operation upon start of out-patient treatment, I focused on the extent of movement and muscle strength of the operated lower extremity. According to acquired results, I established a short-term physiotherapeutic concept, which was subsequently applied in course of 10 joint physiotherapeutic interventions. The following methods were used for therapy: positioning, cryotherapy, vascular and breath gymnastics, techniques of soft tissues, ball facilitation, AFS (after-facilitation stretching), active exercise with an overball, theraband, exercise in closed and open cinematic chains. During 10 joint therapeutic interventions, I conducted continuous control measurements of the extent of movement and muscle strength of the operated lower extremity. In course of the last therapeutic intervention, I carried out an exit exam and based on it, I set up a long-term physiotherapeutic plan. Results: Based on the carried-out short-term physiotherapeutic plan, the full extent of the movement was returned, muscle strength increased, gait stereotype improved, muscle shortening was removed and instability reduced. Conclusion: Thanks to a timely and intensive application of physiotherapeutic methods in post-operative treatment of the patient, the full extent of the movement was achieved, muscle strength improved along with balance and minimisation of possible consequences after the operation of the anterior cruciate ligament of the knee joint.
