Quality of life of children and adolescents with diabetes mellitus

Abstract:

ABSTRAKT Cíl: Diabetes mellitus 1.typu je chronické onemocnění charakterizované zvýšenou hladinou cukru v krvi. V České republice je nyní asi 3300 dětských diabetiků a tento počet se zvyšuje každým rokem o 270-300 dětí. Hlavním cílem práce bylo zjistit kvalitu života u dětí a mladistvých diabetiků ve věku 8-18 let. Hodnocení kvality života je pro děti a dospívající důležité, aby bylo možné posoudit, jak diabetes a jeho náročný léčebný režim ovlivňuje různé oblasti života ? tělesné zdraví, rodinné vztahy, školní a sociální aktivity a podobně. Metodika: Výzkum proběhl od září 2010 do února 2011 v šesti diabetologických ambulancích Moravskoslezského kraje. Byly použity standardizované dotazníky o pediatrické kvalitě života PedsQL? 4.0 a PedsQL? 3.0 Modul Diabetes, které byly doplněné o demografické údaje a hodnotu glykovaného hemoglobinu. Součástí dotazníků byl informovaný souhlas pro rodiče, kteří tvořili druhý vzorek respondentů. Analýza získaných dat byla provedena metodou párového t-testu, t-testu pro dva výběry a analýzy rozptylu ANOVA na hladině významnosti 0,05. Výsledky: Diabetické děti hodnotily kvalitu života lépe než jejich rodiče na obou hodnotících škálách. Diabetické děti hodnotily kvalitu života velmi podobně jako děti zdravé. Bylo zjištěno, že věk ovlivňuje u diabetických dětí především oblast emocionálního zdraví. Trvání onemocnění ovlivňuje rovněž oblast emocionálního zdraví, dále školní fungování, oblast ?symptomy diabetu? a ?starosti?. Dívky hodnotily svou kvalitu života hůře než chlapci, ale statisticky významný vztah byl prokázán pouze v oblasti emocionálního zdraví a starostí s diabetem. Děti s horší kompenzací onemocnění hodnotily kvalitu života hůře než děti s výbornou kompenzací onemocnění. Závěr: Porovnáním výsledků s jinými studiemi lze usuzovat, že diabetické děti, dospívající a jejich rodiče vnímají kvalitu života velmi podobně jak u nás, tak i v zahraničí.

Abstract:

ABSTRACT Objective: Type 1 diabetes mellitus is a chronic disease characterized by increased level of sugar in blood. There are approximately 3300 of diabetic children in the Czech Republic now from and their number increases each year by 270-300 children. The main objective of my work was to determine the quality of life in children and adolescents with type 1 diabetes aged 8-18 years. Evaluation of quality of life (QOL) is vital for children and adolescents with diabetes in order to examine how diabetes and its complicated and demanding treatment regimen affects their various life domains ? physical health, family relationships, school, social activities, etc. Methodology: Research work proceeded since September 2011 till February 2011 at six Diabetology outpatient departments in Moravian-Silesian region. For this research were selected by standardized questionnaires on quality of life of the pediatric version of the PedsQL 4.0 and the PedsQL 3.0 Diabetes Module, which were supplemented with demographic data and the value of glycated hemoglobin. Part of the questionnaire was informed consent for parents who formed the second sample of respondents. Analysis of data was performed using paired t-test, t-test for two samples and analysis of variance ANOVA at significance level 0.05 . Results: Diabetic children evaluated the quality of life better than their parents on both assessment scales. Diabetic children evaluated the quality of life very similar to healthy children. It was found that age influences especially emotional part of evaluation. The duration of the disease also affects the area of emotional health, as well as school functioning, the area "Diabetes symptoms" and "Worries". Girls evaluated their quality of life worse than boys, but statistically significant relationship was demonstrated only in the area of emotional health and worries about diabetes. Children with poor metabolic control of disease evaluated the quality of life worse than children with an excellent compensation disease. Conclusion: By comparing the results with other studies suggest that diabetic children, adolescents and their parents perceive the quality of life in much the same as at home and abroad.
