Effect of relieving knee brace on the dynamics in stance phase of walking in patients with knee osteoarthrosis

Abstract:

Diplomová práce se zabývá vlivem odlehčující ortézy UnloaderOne? na dynamické parametry stojné fáze chůze u pacientů s unilaterální gonartrózou. V teoretické části jsou uvedeny základní poznatky o kolenním kloubu, osteoartróze, terapii a chůzi. Pro záznam reakční síly ve stojné fázi chůzového cyklu byly použity dvě silové plošiny Kistler 9286AA. Do experimentální skupiny bylo zařazeno 6 pacientů s unilaterální gonartrózou středního stupně (průměrný věk 68,3 ? 4,4 let; výška 170,5 ? 11,7 cm; hmotnost 83 ? 15,9 kg), kontrolní skupina obsahovala 6 zdravých probandů podobného věku a tělesné konstituce. Obě skupiny byly měřeny pomocí dynamické analýzy chůze na začátku studie, po 3 měsících aplikace ortézy a 3 měsíce po skončení její aplikace. Při porovnání kontrolní a experimentální skupiny před aplikací ortézy jsme zjistili významný rozdíl (p ? 0,001) v délce trvání stojné fáze chůze a celkovém silovém impulzu vertikální složky reakční síly. Po jednorázové aplikaci ortézy jsme zaznamenali na nepostižené končetině významný rozdíl (p ? 0,05) u minima a 2. maxima vertikální složky reakční síly. Po 3 měsících nošení ortézy jsme zjistili významný rozdíl (p ? 0,05) ve velikosti silových impulzů v brzdící i akcelerační fázi a v celkové délce trvání stojné fáze chůze. Z výsledků vyplývá, že ortotická intervence ovlivňuje dynamické parametry ve stojné fázi chůze.

Abstract:

The master thesis deals with the effect of the knee support brace UnloaderOne? on the dynamic parameters of stance phase of walking concerning patients with unilateral gonarthrosis. The theoretical part includes the basic facts about the knee joint, ostearthrosis, therapy and walking. For measuring the reaction force in the stationary phase of the walking cycle two Kistler type 9286AA force plates were used. The experimental group consisted of 6 patients with the second stage unilateral gonarthrosis (average age 68, 3 ? 4,4 years; height 170,5 ? 11,7 cm; weight 83 ? 15,9 kg), the testing group, on the other hand, consisted of 6 healthy probands of similar age and physique. Both groups were measured by means of the dynamic analysis of walking - at the beginning of the research, three months after the knee support brace application and 3 months after the application was finished. When the both groups before the application of the brace, the testing and experimental one, were compared a great divergence (p ? 0,001) - concerning the duration of the stance phase of walking and also the total force impulse of the reaction force vertical unit ? was discovered. After one-time application of the brace we have found a significant divergence (p ? 0, 05) on the healthy limb concerning the minimum and the 2nd maximum of the reaction force vertical unit. Three months after the brace application we have discovered a significant divergence (p ? 0, 05) concerning the intensity of the force impulse in both, deceleration and acceleration phase, and also the total duration of the stance phase of walking. According to the findings, the orthotic intervention influences the dynamic parameters of the stance phase of walking.
