The influence of the number of platelets in the examination of aggregations on the Multiplate device

Abstract:

Úvod: Funkce krevních destiček je vyšetřovaná v klinických hematologických laboratořích na agregačních analyzátorech. Mezi tyto analyzátory patří například Multiplate, Apact 4004 nebo Thrombomate XRA. Analyzátor Multiplate je založený na principu impedanční agregometrie, zatímco Apact 4004 a Thrombomate XRA jsou založeny na principu světelné transmisní agregometrie. Cíle práce: Cílem práce bylo sledovat vliv počtu krevních destiček na výsledek agregace u skupiny trombocytopenických pacientů s funkčními krevními destičkami a také zdravých dárců na přístroji Multiplate. Dále bylo cílem porovnat agregabilitu krevních destiček na analyzátorech Apact 4004 a Thrombomate XRA. Metodika: Byly vyšetřeny agregace krevních destiček na analyzátoru Multiplate po stimulaci ADP, COL, ASPI a TRAP u celkem 9 vzorků plné krve odebraných do hirudinových zkumavek Sarstedt. V rámci porovnání analyzátorů Apact 4004 a Thrombomate XRA bylo vyšetřeno celkem 33 vzorků po stimulaci ADP, COL, EPI, AA a RIS. Vzorky byly odebírány do zkumavek Sarstedt s pufrovaným citrátem sodným a byla vytvořena plazma bohatá na krevní destičky a plazma chudá na krevní destičky. Výsledky: Při počtu krevních destiček pod 146 x 109/L, resp. 144 x 109/L dochází k ovlivnění agregability. Mezi výsledky analyzátorů Apact 4004 a Thrombomate XRA nebyl zjištěn statisticky významný rozdíl. Závěr: Jedním z cílů této práce bylo najít tzv. cut-off hodnotu. V našem případě se jedná o počet destiček, kdy je pravděpodobnost snížení agregability destiček z důvodu nikoliv jejich poruchy, ale sníženého počtu. Z grafického vyhodnocení dat byla vypočítána průměrná kritická hodnota počtu krevních destiček 146 x 109/L, resp. 144 x 109/L. Dalším cílem bylo porovnat agregabilitu krevních destiček na analyzátorech Apact 4004 a Thrombomate XRA. Statistické a grafické vyhodnocení ukazuje na dobrou korelaci výsledků mezi přístroji Apact 4004 a Thrombomate XRA.

Abstract:

Introduction: Platelet function is examined in clinical haematology laboratories on aggregation analysers. These analysers include Multiplate, Apact 4004 or Thrombomate XRA. The Multiplate analyser is based on the principle of impedance aggregometry, while the Apact 4004 and Thrombomate XRA are based on the principle of light transmission aggregometry. Aims: The aim of the study was to investigate the effect of platelet count on the aggregation outcome in a group of thrombocytopenic patients with functional platelets and healthy donors on the Multiplate device. Furthermore, the aim was to compare platelet aggregability on the Apact 4004 and Thrombomate XRA analyzers. Methods: Platelet aggregation on the Multiplate analyzer after stimulation with ADP, COL, ASPI and TRAP was examined in a total of 9 whole blood samples collected into Sarstedt hirudin tubes. A total of 33 samples were analyzed after stimulation with ADP, COL, EPI, AA and RIS on the Apact 4004 and Thrombomate XRA analyzers. Samples were collected into Sarstedt tubes with buffered sodium citrate and platelet rich plasma and platelet poor plasma were generated. Results: Platelet counts below 146 x 109/L or 144 x 109/L affect aggregability. There was no statistically significant difference between the results of the Apact 4004 and Thrombomate XRA analyzers. Conclusion: One of the objectives of this work was to find the so-called cut-off value. In our case, it is the number of platelets, where the probability of reduced aggregability of platelets is due not to their failure, but to their reduced number. From the graphical evaluation of the data, the average critical value of platelet counts was calculated to be 146 x 109/L and 144 x 109/L, respectively. Another aim was to compare platelet aggregability on Apact 4004 and Thrombomate XRA analyzers. Statistical and graphical evaluation shows a good correlation of results between the Apact 4004 and the Thrombomate XRA.
