Co-creation and cooperation in the psychotherapeutic process

Abstract:

Předkládaná práce se věnuje dvěma tématům. Uchopení psychoterapeutického procesu jako společného díla klienta a psychoterapeuta. A hledání způsobů, jak tento proces zachytit a následně prakticky podpořit. V první kapitole se věnuje dialogismu jako specifickému paradigmatu pro psychoterapii a její výzkum. Identifikovány byly dva základní proudy, kterými se výzkum psychoterapeutického procesu v rámci dialogismu doposud ubíral. První klade důraz na analýzu sezení dvojice a větší zaměření na intrapsychické procesy. Druhý se zaměřuje na analýzu procesu za přítomnosti více účastníků a důraz na analýzu polyfonie. Teorie dialogismu, a některé metodologické postupy z ní plynoucí, byly inspirací pro design případové studie. Té je věnována druhá kapitola. Studie je pojatá jako na důkazech založená systematická případová studie. Téma psychoterapeutické spolupráce posouvá k obecnějším konceptům pracovní aliance a responzivity v psychoterapii. Ve studii jsou souladu s východisky zakotvenými v dialogismu chápány pracovní aliance i responzivita jako dynamické a spoluutvářené dílo vznikající za přispění psychoterapeuta i klienta. Studie analyzuje případ, ve kterém došlo dle kvantitativních i kvalitativních ukazatelů ke zlepšení v oblasti symptomatologie i v oblasti subjektivního prožívání klientky. Navzdory tomu se klientka rozhodla jednostranně a předčasně psychoterapii ukončit. Ve studii se snažíme ilustrovat, jak mohou být různé procesové proměnné v psychoterapii provázány a vzájemně spolu interagovat. Ukazujeme, že aliance není statickou veličinou, ale spíše dynamicky se proměňující kvalitou, o které je nutno vyjednávat v závislosti na vynořujícím se kontextu. Fungující konfigurace pracovní aliance z počátku psychoterapeutické spolupráce se v pozdějších fázích může stát konfigurací brzdící. Poslední kapitola je praktickým vyústěním, které nabízí přehled možných praktických postupů jak vzájemnou spolupráci klienta a psychoterapeuta v procesu psychoterapie podpořit. Konkrétně se věnuje práci s preferencemi klientů, rutinnímu monitorování psychoterapie přímo s klientem a konceptu terapeutické zóny proximálního vývoje jako nástroje pro okamžitou reflexi psychoterapeutického procesu.

Abstract:

The present work focuses on two topics. Understanding the psychotherapeutic process as a joint work of client and psychotherapist. And the search for ways to capture and then support this process in practice. The first chapter explores dialogism as a specific paradigm for psychotherapy and its research. Two main streams of research on the psychotherapeutic process within dialogism have been identified. The first emphasizes the analysis of individual therapy sessions and a greater focus on intrapsychic processes. The second focuses on the analysis of the process in the presence of multiple participants and an emphasis on the analysis of polyphony. The theory of dialogism, and some of the methodological approaches derived from it, inspired the design of the case study. This is the focus of the second chapter. The study is conceived as an evidence based systematic case study. It moves the theme of psychotherapeutic collaboration towards the more general concepts of working alliance and responsiveness in psychotherapy. Consistent with the assumptions embedded in dialogism, the study understands both working alliance and responsivity as dynamic and co-created work emerging with the input of both a psychotherapist and a client. The study analyses a case in which, according to quantitative and qualitative indicators, was an improvement in the symptomatology and in the subjective experience of the client. Despite this, the client decided to terminate psychotherapy unilaterally and prematurely. In this study, we aim to illustrate how different process variables in psychotherapy can be interrelated and interact with each other. We show that alliance is not a static variable, but rather a dynamically changing quality that must be negotiated depending on the emerging context. A working alliance configuration at the beginning of a psychotherapeutic collabora-tion became a hindering configuration in later stages. The last chapter is a practical outcome that offers an overview of possible practical approaches to support the mutual collaboration between client and psychotherapist in the psychotherapy process. Specifically, it discusses working with client preferences, routine outcome monitoring with the client, and the concept of the therapeutic zone of proximal development as a tool for immediate reflection on the psychotherapeutic process.
