Analýza výdeje chorobu-modifikujících antirevmatických léků při léčbě revmatoidní artritidy v okrese České Budějovice

Analysis of the distribution of disease-modifying antirheumatic drugs in the treatment of rheumatoid arthritis in the district of České Budějovice

Abstract:

Rigorózní práce se zabývá problematikou revmatoidní artritidy (RA). Cílem práce bylo zhodnotit spotřebu léčivých přípravků ze skupiny chorobu-modifikujících antirevmatických léků (Disease-modifying antirheumatic drugs, DMARD) na základě jejich výdeje v lékárně Medipont Plus v Českých Budějovicích v pětiletém období od 1. 1. 2018 do 31. 12. 2022. Spotřeba léčiv byla hodnocena na základě předepsaných definovaných denních dávek (DDD) jednotlivých ATC skupin a na základě celkové úhrady léčivých přípravků. Získaná data o spotřebě DMARD používaných v terapii RA ukázala, že rozložení vydaných léčiv korespondovalo s odbornými doporučeními České revmatologické společnosti pro farmakologickou léčbu revmatoidní artritidy. Za hodnocené pětileté období let 2018–2022 byla nejvyšší spotřeba podle DDD zjištěna u konvenčních syntetických chorobu-modifikujících antirevmatických léků (Conventional synthetic disease-modifying antirheumatic drugs, csDMARD) u ATC skupiny – methotrexát (MTX). MTX je lékem volby, je nasazován v první linii léčby RA. Spotřeba dle DDD u skupiny csDMARD odpovídala i nejvyšší poskytnuté úhradě ze zdravotního pojištění v této skupině csDMARD, kdy za pětileté období let 2018–2022 byla poskytnuta celková úhrada v částce 3 459 573,8 Kč za léčivou látku methotrexát ze skupiny csDMARD, což bylo v přepočtu 47 % ze součtu souhrnné úhrady v rámci celé skupiny csDMARD. Dále proběhlo vyhodnocení věku a pohlaví pacientů užívajících léčivé přípravky skupiny csDMARD používaných při léčbě RA. Celkový poměr žen k mužům léčených některým z csDMARD léků za celé pětileté hodnocené období let 2018–2022 byl zaokrouhleně 2,7:1. Tento výsledek odpovídal faktu, že onemocnění je častější u žen s poměrem žen k mužům 3:1, který najdeme v odborné literatuře a který uvádí i Česká revmatologická společnost. V rámci práce bylo také vypracováno Doporučení pro poskytnutí informací při výdeji léčiv skupiny csDMARD při terapii RA v běžné lékárenské praxi.

Abstract:

The rigorous thesis deals with the issue of rheumatoid arthritis (RA). The aim of the work was to evaluate the consumption of medicinal products from the group of disease-modifying antirheumatic drugs (DMARD) based on their dispensing at the Medipont Plus pharmacy in České Budějovice in the five-year period from 1. 1. 2018 to 31. 12. 2022. Drug consumption was assessed on the basis of the prescribed defined daily doses (DDD) of individual ATC groups and on the basis of the total reimbursement of medicinal products. The obtained data on the consumption of DMARDs used in RA therapy showed that the distribution of dispensed drugs corresponded to the expert recommendations of the Czech Rheumatology Society for the pharmacological treatment of rheumatoid arthritis. During the evaluated five-year period of 2018–2022, the highest consumption according to DDD, was for conventional synthetic disease-modifying antirheumatic drugs (csDMARD) in the ATC group – methotrexate (MTX). MTX is the drug of choice, it is used in the first line of RA treatment. The consumption according to DDD in the csDMARD group also corresponded to the highest reimbursement provided from health insurance in this csDMARD group, when for the five-year period of 2018–2022, a total reimbursement of CZK 3 459 573,8 was provided for the medicinal substance methotrexate from the csDMARD group, which was recalculation of 47 % of the total reimbursement within the entire csDMARD group. In addition, the age and gender of patients using medicinal products of the csDMARD group used in the treatment of RA were evaluated. The total ratio of women to men treated with any of the csDMARD drugs for the entire five-year evaluation period of 2018–2022 was rounded to 2,7:1. This result corresponded to the fact that the disease is more common in women, with a ratio of women to men of 3:1, which can be found in the professional literature and which is also reported by the Czech Society of Rheumatology. As part of the work, Recommendations for providing information during the dispensing of drugs of the csDMARD group during RA therapy in ordinary pharmacy practice were also developed.
