Controlled hypothermia in patients after CPR in prehospital and hospital care

Abstract:

Abstrakt Srdeční zástava je závažná událost, i po obnovení krevního oběhu však zůstává prognóza pacienta nejistá. Během ischemicko-anoxické fáze při samotné zástavě srdce se spouštějí mechanismy, které poškozují mozek a další orgány ještě několik hodin po úspěšné resuscitaci. Řešením, které brání vzniku těchto škodlivých procesů a zároveň zlepšuje neurologický výsledek u nemocných po resuscitaci, je používání metody řízené mírné hypotermie. Teoretická část bakalářské práce seznamuje s metodami chlazení, protektivními účinky mírné hypotermie, komplikacemi, které při indukci hypotermie mohou nastat a s výsledky studií, zabývajících se metodou mírné hypotermie. Součástí práce je také navrhovaný postup při indukci mírné hypotermie. V praktické části práce je uvedena kazuistika nemocného, u kterého byla zahájena terapeutická hypotermie již v přednemocniční péči, v kazuistice je zachycena návaznost péče v nemocničním zařízení a další doléčení pacienta. Indukce mírné hypotermie byla tématem mnoha studií, na základě jejichž výsledků byly doporučeny nejvhodnější metody a postupy ochlazování v přednemocniční péči i v nemocničních podmínkách. Uvedená kazuistika však odhaluje určité nedostatky v užívání mírné hypotermie v praxi. Nejzávažnějším nedostatkem se zdá být chybění standardizovaného postupu k ochlazování nemocného a s tím související nepřehledná dokumentace o průběhu a fázích mírné hypotermie.

Abstract:

Abstract Cardiac arrest is a serious event and even after the revival of blood circulation the patient?s prognosis remains insecure. During the ischemic-anoxic phase of the cardiac arrest mechanisms damaging the brain and other organs even several hours after a successful resuscitation are started. A method of controlled mild hypothermia is a solution that prevents the activation of the harmful processes and also improves the neurological results after resuscitation. The theoretical part of the bachelor thesis introduces hypothermic methods, protective effects of mild hypothermia, possible complications that may arise during the induction of hypothermia, and results of studies dealing with method of mild hypothermia. In the practical part is presented a casuistry of a patient who was treated with therapeutic hypothermia in the pre-hospital care. The casuistry shows the continuity of hospital care and aftercare. The induction of mild hypothermia has been a subject of many theses based on which the best methods and procedures were recommended in both pre-hospital and hospital care. The presented casuistry shows some defects of mild hypothermia induction in practice. A lack of standardized procedure during hypothermia and unclear documentation about the course and phases of mild hypothermia seem to be the most serious defects.
